INITIAL NOTIFICATION

  • Is injury severe enough require emergency medical services/ambulance?

  • Call 911 or site specific number immediately.

  • Are you at mishap location?

  • Add location
  • Use "locate me" function to obtain address.

  • Name of injured employee:

  • Select date

  • Division Safety Manager notified by phone. Leave voice mail if no answer. Division Safety Manager will notify other personnel on notification matrix.

  • Notify project superintendent and project manager. Leave voice mail if no answer.

  • Notify General Contractor (or client for owner-direct work).

MISHAP RESPONSE

  • Are you able to immediately report to the mishap scene?

  • Notify Division Safety Manager by phone. Leave voice mail if no answer. Await further instructions.

  • CARING FOR INJURED EMPLOYEE IS TOP PRIORITY! Assist with first aid if needed.

  • Secure scene to preserve evidence. Physical evidence will be secured and a paper Chain of Custody established.

  • Determine if there are witnesses. If there are multiple witnesses, they should be separated to avoid collaboration.

  • Take photos of scene and physical evidence.

  • Add media

  • Have foreman fill out applicable parts of accident report.

  • Have witness(es) write out statements. Statements will be turned in to foreman, superintendent or project manager.

  • Notify Division Safety Manager to provide update. Leave voice mail if no answer. Division Safety Manager will provide update to other personnel on notification matrix.

ACCOMPANY INJURED EMPLOYEE TO MEDICAL CARE FACILITY

  • Is the injury work related?

  • Are you being allowed to accompany injured employee for treatment?

  • Fill out workers' compensation forms.

  • Ensure employee is tested for drugs and alcohol.

  • Provide sample of modified duty jobs to provider.

  • Review OSHA Recordkeeping Handbook for classification of injury.

  • Inform facility that HIPPA does not apply to workers' comp cases. Do not argue is admittance is still denied.

  • Stay with injured employee until family member arrives or employee is released.

DISCHARGE OF INJURED EMPLOYEE

  • What is the injury classification?

  • Obtain full duty release from doctor.

  • What is making it recordable?
