    Project Health and Safety Inspection Checklist

  • To be completed by Project Supervisor or Nominated person weekly from the start of construction work on site. Completed forms are to be kept readily available (preferably on site) for the duration of construction works, after which time shall form part of the CDM Health & Safety file.

Pipe Stringing

  • Any H&S actions identified shall be entered onto Audit/Inspection Action Log and brought to the urgent attention of the appropriate personnel as soon as possible. Contractor/Project Manager to be notified of actions required/taken.

  • S = Satisfactory, U = Unsatisfactory, O = Observation, NA = Not Applicable

  • Is there a Method Statement/ Risk Assessment available on site for the activity? Has it been briefed out and signed for?

  • Are the Emergency Procedures available on site? Are all operatives aware of them?

  • Have all Operatives been inducted to the Project and received E& US training?

  • Are the welfare facilities adequate and is there appropriate first aid provision on site?

  • Numbers of Appointed persons/ First Aiders, location/ contents of kit

  • Is the correct P.P.E. for the task being worn? (Check condition, age, storage, training)

  • Is the site tidy and free from trip hazards and is there safe access and egress available for all?

  • Are all operatives competent to perform the task? (Check CITB, CSCS cards, training records etc.) Crane driver, banksmen, slingers.

  • Are pipe delivery drivers in possession of the latest version of the traffic management plan?

  • Are pipes adequately secured to the trailer or pipe carrier? (Chocked, strapped down etc.)

  • Is there a safe means of access up to the pipe for inspection and connection of lifting equipment?

  • Is there a one-way system in place to allow vehicles to exit the spread or are they required to turn round? Is there adequate space for this and banksman used?

  • Are up to date certification and inspection results available for the crane and lifting accessories?

  • Are weekly checks recorded for the crane/ lifting appliance? Are visual inspections of slings, lifting shoes etc. carried out by a competent person?

  • Is the lifting appliance on a firm and level base?

  • Are tag lines attached at each end of the pipe to control it during the lift?

  • If the vehicle is required to travel while carrying the pipe, is this done in a controlled way to prevent the load swinging out of radius?

  • Are safe manual handling techniques used during the moving of skids?

  • Are the pipes held securely by the skids and wedges; are they in suitable condition?

Vacuum Lifting

  • Is the driver competent to use the vacuum lift?

  • What daily/ weekly checks are carried out on the vacuum? Are these recorded?

  • Is certification for the vacuum available?

  • What visual warnings are in place to inform the driver when it is safe to lift?

  • Is a test lift done each day to ensure the vacuum will hold if the power is lost? Is this recorded?

  • How is the centre of gravity of the pipes marked? Does this require climbing onto the trailer and could it be done before delivery?
