  • Any H&S actions identified shall be entered onto Audit/Inspection Action Log and brought to the urgent attention of the appropriate personnel as soon as possible. Contractor/Project Manager to be notified of actions required/taken.

  • S = Satisfactory, U = Unsatisfactory, O = Observation, NA = Not Applicable

  • Are risk assessments carried out and do they include the use of vibrating equipment and the precautions to be taken to reduce the risk?

  • Have individuals who use vibrating equipment been Identified? If so:- have they been informed of the associated risk

  • Is new equipment assessed prior to purchase? If so are these assessments used as a selection criterion?

  • Is new equipment purchased in line with a company standard?

  • Does the purchase policy take account of vibration along with other health & safety issues? And does the equipment comply with British and European Standards?

  • Is current equipment maintained according to manufactures instructions and are records of maintenance retained and available?

  • Is the equipment that is hired of a similar specification to that which is purchased and are checks carried out to ensure that the equipment meets the required specification?

  • Is work with vibrating equipment limited as far as reasonably practicable?

  • Where the potential exists for exposure limits could be exceeded, have measures been put in place to ensure adequate rest periods between operating vibrating equipment.

  • Is there a Health Surveillance programme for monitoring exposure to vibration?<br><br>If so has an annual HAVS vibration questionnaire been issued to the relevant employees?<br><br>Are completed questionnaires returned to the Occupational Health Adviser (Nurse)? <br><br>And where appropriate is a medical examination undertaken using the Stockholm scale?

  • In order to filter out individuals likely to be susceptible to HAVS, Is Health Screening carried out prior to, recruitment, Internal transfer?

  • Where HAVS is diagnosed are managers informed by the Medical Advisor of actions required? <br><br>And any recommended restrictions for limited exposure implemented? <br><br>Are RIDDOR F2508 Forms completed and forwarded to the Enforcing Authority?

  • Are employees who use vibrating equipment trained (Vibration Training Package)? <br><br>If so does the training extend to include their managers? <br><br>Are training records retained? Are they kept up to date and available? <br><br>And is refresher training carried out at three yearly intervals?

  • Are existing and new employees who may be exposed to work involving the use of vibrating equipment made aware of the Vibration Management Procedure? <br><br>And the purpose and content of health surveillance programme?
