  • Have all personnel involved in the task read and signed onto SWMS?

  • Are personnel wearing the correct PPE as per SWMS requirements?

  • Has a pre-start inspection on the equipment been completed for the day?

  • Has operator of the equipment had the correct training and deemed competent?

  • If slinging loads, does the operator hold a current Dogging certificate? (DG)

  • Has the worksite been inspected to identify lift paramaters, potential hazards and suitable position for machine?

  • Are underground services identified where the lifting equipment is going work and is the travel path free from obstruction, level, and safe for travel? (Uneven ground and underground services, i.e. pits, soak wells)

  • Has an exclusion zone been developed and delineated appropriately using flagging/bollards/caution tape?

  • Has a lift study been performed to select the correct type and lifting capacity machine to lift within safe operating limits at the required working radius?

  • Is all lifting equipment selected, inspected and quarterly tagged by a dogman/rigger?

  • Are tag lines used to control the loads?

  • Are all personnel in the lift vicinty wearing a hard hat?

  • Is the dogman wearing gloves at all times when working with loads?

  • Are communication procedures/protocols established between all work parties?
