1.0 School Responsibility

3.0 Housekeeping

  • Food and drink: ( Are not stored or consumed in laboratories - look for evidence of non conformance and deal with immediately.

  • Doorways - Escape Routes and Floors: Clear ( Of clutter and trip hazards e.g boxes , waste containers etc)

  • Benches: Orderly ( adequate free space available )

  • Fume Cupboards: ( Free of clutter and large equipment unless unless designated, there are no open waste containers not in use )

  • Shelves: Orderly with no large , regularly used items above shoulder height)

4.0 Gas Cylinders

  • Must be in use ( Not stored in the laboratory)

  • Are correctly secured ( ie individual chains) and protected from damage.

  • Have correct regulators and fittings ( if cylinders or regulators are greater than 10 years old they need to be checked and tested. See information on neck of cylinder.

  • There is a cylinder trolley available for use.

5.0 Pressure vessels

  • Details available ( ie maximum pressure and operating temperature; hazard class etc)

  • Appropriate testing and maintenance is conducted.

  • If the pressure vessels are registered with Worksafe , the required inservice inspections are conducted and the licences kept current ( e.g for large autoclaves,air compressors) Provide specific details to the OHS Consultant if not sure to ascertain requirements.

6.0 Electrical Safety

  • There are sufficient general purpose power outlets (GPO'S) for the equipment.

  • Double adapters and piggy back plugs are not in use. ( banned from use at Monash)

  • Portable electrical equipment has been tested and tagged within the last 12 months)

  • Refrigerators and freezers :(All electrics are on the outside)

7.0 chemical Storage and labelling.

  • Storage on open shelving: All containers are less than 5 kg or L in size.

  • On shelves greater than 1.5 m high all glass containers are less than 1 kg or L size

  • All containers in cupboards are less than 4 kg or L in size.

  • Flammable Chemicals ( including waste) <br>Flammables Cabinet: The self closing mechanism do the doors are operating correctly.

  • Contents of the Flammables cabinets are with its load limits ( 100 L max for 4 person lab ca 60 Sq M)

  • Not more than 10 L of class 3 , flammable liquid is to be stored on open shelves / lab ( 60 sq M)

  • Corrosive chemicals: DG Class 8 - Are stored in a corrosives cabinet or vented cabinet.<br>

  • Decanted chemicals: <br>No food containers are in use for the storage of chemicals.<br>Are properly labelled ,segregated and stored.<br>All reused containers have the label removed or completely covered.

  • Controlled Substances: ( Poisons, Drugs,Precursors)<br>Permits and procedures are in order and they are properly secured. ( e.g.locked cupboard)

  • Storage within fridges/freezers:<br>Segregated,properly labelled, secondary containment.

  • Highly dangerous chemicals/ time/temperature sensitive chemicals are monitored to ensure they do not deteriorate or are kept past expiry date ( discuss with lab supervisor)

  • Biohazardous materials: <br>Are appropriately isolated as required<br>Correct labelling <br>Appropriate disinfectants are available<br>Appropriate safe work instructions are in use for biosafety/biosecurity ( autoclave, chemical disinfection,decontamination)

  • Other DG Classes e.g 4.3,5.1,6.1:<br>Are correctly stored and within load limits.

  • Smelly/highly odorous compounds:<br>stored in ventilated cupboard or refrigerator as appropriate.

  • Non Hazardous chemicals:<br>Correct labelling and Container size less than 4 kg.

  • Cryogenic liquids: ( Liquid Nitrogen,helium)<br>Are stored in an appropriate container; If container s greater than 60 L , a Low oxygen monitor is in the lab. <br>Ensure adequate floor protection. <br>Users have received training in safe handling. ( ask occupants/supervisor)<br><br><br>

8.0 Waste Management:

  • Wastes are disposed of regularly and volumes are kept to a minimum.<br>Note: Request prompt action if there s evidence of a build up of waste in the laboratory.

  • Hazardous waste is clearly labelled with the waste description,biohazard symbol if required and date.

  • Correct DG class, adequate labelling and in proper container. ( 10 L max for liquids)

  • Biohazardous waste and sharps: ( In correct container and properly labelled)

9.0 Distillation set ups:

10.0 Ionising radiation hazards

10.1 Unsealed sources

  • Documentmented risk assessments and safe work procedures for use of ionizing radiation are available

  • Work surfaces are impervious (wooden benches and tiled surfaces are unsuitable)

  • Hand washing facilities are available

  • Appropriate shielding material is available

  • Secondary containment is available and utilized (ie spill trays)

  • All sources of ionizing radiation are labelled and stored in secure locations

  • All storage locations of ionizing radiation are labelled

  • Appropriate monitoring equipment is available

  • Decontamination/emergency procedures are clearly displayed

10.2 sealed sources

  • Documentmented risk assessments and safe work procedures for use of ionizing radiation are available

11.0 Ultraviolet radiation

12.0 Lasers

  • Access to Class 3 and Class 4 lasers is controlled

  • Beam stops are installed and access to the beam is restricted

  • Aopropriate fail-safe devices to prevent accidental exposure are in place

  • Documented risk assessments and safe work procedures for use of lasers are available

  • Warning signs are clearly displayed

  • Wavelength-specific eye protection is available for laser operators

13.0 Environmental issues

  • Chemical containers are cleaned prior to Recycling or disposal<br>- triple rinsed with labels and lids removed before placement in the bottle and can recycling bins<br>- containers with oily or persistent residues are washed with detergent and labels removed before placing in the bottle and can recycling bins

  • Contaminated glassware that is not cleaned as above is treated as prescribed industrial waste

  • Ther is no evidence of incorrect disposal down sinks, staining, odorous sinks etc

  • There are no odours, dust or smoke in area

  • All service areas, where chemicals are used and silenced or where wastes are stored are suitably bunded/protected from leakage into storm water or sewer drains

  • All electrical equipment, including computers. Are switched off when appropriate
