Outstanding items on the Register of Non-Conformance have a risk level ascertained and responsible person allocated

Completed items in the Register of Non-Conformance have actions taken and date rectified recorded

Hazard / Near Miss forms have been completed and sent to the YMCA Safety Department for any new hazards that have been identified and unable to be eliminated

Are there any outstanding ations required from the previous WH&S audit?

All incident report forms have been sent to YMCA Administration

Instruction & Training

Any new employees have had inductions completed

Any new staff have been added to the Staff Register

All staff have completed a General Induction

All staff have completed an OHS Induction

All staff have received Fire and Emergency Evacuation Training within the past 12 months and have signed the training register (Form 255) and had the date of training added to the Staff Register

A Fire Evacuation Drill has been completed in the last 12 months and an Emergency Drill Observation Record (Form 369) has been completed
