Audit Checklist

Section 1: Personnel (Section 3.2.1.2(3) - TF Responsibilities)

  • Accredited Person(s) (AP)

  • Deputy Transitional Facility Operator (DTFO)

Section 2: Record Keeping (Section 3.6 - Record Keeping)

  • Records for sterilisation are available, this includes thermocouple reading, biological indicator report and temperature of the chamber during a treatment cycle

  • Select all records that are kept

Section 3: Hygiene Management (Section 3.7 - Hygiene Management)

  • PPE provided for staff

  • MPI approved disinfectant is used

  • Disinfectant footpads or footbaths are placed at all entries/exits

  • Floors and other surfaces are washed clean and disinfected with MPI approved disinfectant

  • All equipment used is checked and tested regularly

Section 4: Staff Training (3.2.1.2(6) - Training)

  • Personal Training Plan

  • Discuss information from MPI e.g. Foot and Mouth; pests

  • Posters and noticeboards containing biosecurity information is displayed

  • Biosecurity awareness training to staff members in weekly toolbox meetings

Section 5: Physical Site Requirements

  • Transitional Facility Operator controls access and security at all times (Section 2.1 - TF Security)

  • Signage: TF signs are located in the facility (Section 2.3 - TF Signage)

  • All uncleared Air and Sea containers are held in a secure manner at all times (Section 4.3.2(1)(a-e) - Operational Requirements for decontamination TFs)

  • The devanning area is constructed of either concrete or asphalt and is in good condition (no cracks or gaps) (Section 4.3.2(1)(a-e) -Operational Requirements for decontamination TFs)

  • A clearance of no less than 1 metre is maintained between non-AP checked containers (Section 4.3.2(1)(a-e) - Operational Requirements for decontamination TFs)

  • A clearance around an entire container of no less than 3 metres from surrounding vegetation, debris and rubbish is maintained at all times (Section 4.3.2(1)(a-e) - Operational Requirements for decontamination TFs)

  • Any open drains within 5 metres of containers are to have approved drain covers (Section 4.3.2(1)(a-e) - Operational Requirements for decontamination TFs)

  • Holding/inspection areas are located indoors (Section 3.3.1(2) - Controlled Areas)

  • The holding/inspection areas have a clearance of no less than 3 metres from cleared and/or domestic goods (or alternatives such as shrink wrapping) (Section 4.3.2(1)(a-e) - Operational Requirements for decontamination TFs)

  • The holding/inspection area, where approved by MPI, have a residual Insecticide Programme (Section 4.3.2(1)(a-e) - Operational Requirements for decontamination TFs)

  • Lighting level: Adequately illuminated (600 lux - 1000 lux) (Section 3.10(1)(a) - Lighting)

  • Receipt, management and transfer of uncleared risk goods (Section 3.3 - Receipt, management and transfer of uncleared risk goods)

  • Segregation of uncleared risk goods is maintained (Section 3.5 - Segregation of uncleared risk goods)

  • Biosecurity waste is placed inside leak proof packaging within a lockable HSTU or lockable bin within a leak proof liner (Section 4.1.1(1)(a or b) - Transport of biosecurity refuse)

Section 6: Vehicles (Section 4.1.1(1-4) - Transport of biosecurity refuse)

  • Trucks are fitted with a drainage system and spillage tanks

  • Insecticide spray and spill kits

  • Washed and cleaned daily

  • If contamination is found in the vehicle, MPI approved disinfectant is used

  • Transport routes are listed in the Operating Manual

  • Transfer of uncleared risk goods must be conducted in a secure and contained manner

Section 7: Pests, Other Organisms, Vermin and Weed Control (Section 3.2.1.4(5) - Pests, Vermin and Weed Control)

  • Bait stations sighted at the facility

  • Weeds not present

  • Birds under control

  • Flies, cockroaches are under control

Section 8: Internal Audit (Section 3.9 - Internal Audit)

  • Site inspection - done monthly

Section 9: Contingency Plans (Section 3.11 - Contingency Plan)

  • Site specific operating manual (to be reviewed annually)

  • Emergency response manual

  • Business Continuity Plan

Section 10: Autoclaves (heat treatment) (Sections 4.4.6/4.4.7 - Sterilisation TFs)

  • Have annual heat treatment chamber calibration

  • Have minimum of two temperature recording probes per chamber (and humidity recording probes where required)

  • Have automatic thermostatic controls with an automatic temperature recorder to record the time and temperature (and humidity where required) (Section 4.2.5 - Heat Treatment TFs)

Section 11: Effluent Waste Treatment (Section 4.4.5 - Effluent Waste Treatment)

  • Effluent generated during bin washing is filtered through 2mm sieves to remove any solids

  • Solids are chemically treated or sterilised on site. Captured and placed in IHSTU for transport and processing in Wellington (Section 4.1.6 - Effluent Waste Treatment)

Section 12: Other

  • MPI is notified immediately if there is a disruption to normal operation (Section 4.1.9 - Disruption to normal operations)

  • TF Operator must report any spillage or leakage of uncleared risk goods to an inspector immediately (Section 3.3.2(3) - Reporting spillage)

  • Biosecurity waste is stored correctly on site until transport to Wellington (Section 4.1.3(1)(a) - Timing requirements)

Section 13: Biosecurity Equipment is available and labelled (Section 4.1.5 - Reusable Equipment)

  • Waste bin (with liner)

  • As the approved Transitional Facility Operator, in accordance with Sections 39 and 40 of the Biosecurity Act 1993. I declare that to the best of my knowledge the above information is true and correct. I understand that failure to complete any of the above information may result in further action being taken by the Ministry for Primary Industries
