C&K Service Profile - This template details a number of items required by law to be displayed. Please ensure your service profile details current information.

Service operations details (these details are recorded on the C&K service profile template): - Days and hours of operations. - Name and phone number of the person who can be contacted to receive a complaint. - Name of educational leader. - Contact details of the regulatory authority.

Name of nominated supervisor (detailed on the C&K service profile template)

Staff sign in and out record, indicating the certified supervisor in charge

Service rating: - Rating for each quality area and the overall rating in the National Quality Standard (NQS). - If a service has yet to be assessed the service must be display: “This service has yet to be assessed against the National Quality Framework”. - If a service was previously assessed under the previous accreditation system and has not been assessed against the NQS, the service must display their quality profile.

If applicable waiver details: - The elements of the NQS or the National Regulations that have been waivered. - The duration of the waiver and whether or not the waiver is a service or temporary waiver.

Most current version of QIP - visitors, families and authorised officers must have ready access.

If applicable, a notice stating that "A child at risk of anaphylaxis is enrolled at the service"
