EVALUATION

  • Are all activities performed in compliance with an accepted JSEA that is still adequate for the current tasks & conditions?

  • Do all persons appear to understand the requirements of the JSEA & have signed off?

  • If persons are not working within two metres of an unprotected open edge from which the person could fall, have control measures been implemented to prevent the person from falling, or if the prevention is not practicable:

  • If the person can not fall more than 2 metres has the risk been considered & if necessary, controls implemented to reduce the risk to an acceptable level?

  • If the person is working above 2 metres, have measures been taken to arrest the workers fall?

  • Is there documented evidence that anchorage points were designed by an engineer or deemed suitable by a competent person?

  • Does the system have a device to absorb the energy of a falling person?

  • Has the system been installed, maintained & used in accordance with the manufacturer or suppliers' instructions, or those instructions given by an engineer or competent person?

  • Is there documented evidence that the system is inspected & tagged by a competent person every month?

  • Do the system components show no evidence of wear or weakness?

  • Is each anchorage point located so that a person can attach to it before they are at risk of falling?

  • Is there enough distance available for a person using the system to fall to prevent the worker hitting an object, the ground or another surface other than a vertical surface?

  • Has it been checked that no part of the system will come into contact with anything that will affect the safe use of the system?

  • Are post fall recovery methods (rescue plan) documented, communicated & practical?

  • Is the system not being used when the employee is working alone?

  • Are there any other risks or issues associated with the activities observed?
