EVALUATION

  • Are all activities performed in compliance with an accepted JSEA that is still adequate for the current tasks & conditions?

  • Do all persons appear to understand the requirements of the JSEA & have signed off?

CHAINS

  • Is there evidence that chains are inspected by a competent person before each use & by a 'Lifting Equipment Specialist' at intervals as recommended by the manufacturer? (usually annually)

  • Is each chain sling supplied with a certificate & an identification tag which includes the name of the manufacturer, the grade of the chain, the nominal chain size, the number of chain legs, the length (reach) of the sling, the rated loads for the sling assembly (WLL) & the angle upon which the rating is based?

  • Are every 20 links marked with the manufacturer's identificatiion & quality grade? (T [grade 80 - high tensile])

  • Are all chains, hooks, links & couplers making up a chain sling of the same grade & WLL? (80 grade or higher)

  • Are all loads within the specified WLL of the chain?

  • Was the chain not:

  • Knotted for shortening purposes?

  • Shortened by using nuts & bolts?

  • Hammered to straighten the links or to force into position?

  • Were loads not set onto chains?

  • Were vehicles/mobile plant prevented from driving over chains?

  • Was protective padding used on chains placed around sharp corners?

  • Was a chain not used if:

  • The links were stretched more than 3%?

  • The links did not move freely?

  • It was kinked or twisted?

  • A link was cracked or as been spot welded other than by the manufacturer?

  • It has been artificially heated or heat-treated?

STEEL WIRE ROPES

  • Is there evidence that ropes are inspected by a competent person before each use & by a 'Lifting Equipment Specialist' at intervals as recommended by the manufacturer? (usually annually)

  • Were ropes unkinked?

  • Had the ropes been exposed to a corrosive substance?

  • Were the ropes suitably oiled or greased to ensure sufficient lubrication?

  • Were reeled ropes mounted on a spindle & securely anchored?

  • Are there any other risks or issues associated with the activities observed?
