EVALUATION

  • Are all activities performed in compliance with an accepted JSEA that is still adequate for the current tasks & conditions?

  • Do all persons appear to understand the requirements of the JSEA & have signed off?

HOOKS

  • Is there evidence that hooks are inspected by a competent person before each use & by a 'Lifting Equipment Specialist' at intervals as recommended by the manufacturer? (usually annually)

  • Are hooks marked with the WLL & manufacturer's identification code?

  • Are hooks fitted with a safety catch?

  • Are hooks to be used in a chain sling at least the same WLL as the chain?

  • Was the hook not used if:

  • It (crane hook) is unable to rotate freely at all times?

  • The hook is bent or distorted?

  • The hook has been subjected to a welding repair?

SHACKLES

  • Is there evidence that shackles are inspected by a competent person before each use & by a 'Lifting Equipment Specialist' at intervals as recommended by the manufacturer? (usually annually)

  • Are shackles marked with the WLL & manufacturer's identification code & quality grade? (S or higher)

  • Are shackles at least the same WLL as the chain, hook & other parts of any sling used?

  • Were nuts and bolts not used in place of the proper shackle pin?

  • Did shackles permanently attached to lifting devices have their pins secured by seizing or mousing?

  • Was any shackle that was bent, deformed, damaged or worn in the crown or pin by more than 10% not used?

SHEAVE BLOCKS

  • Is there evidence that sheave blocks are inspected by a competent person before each use & by a 'Lifting Equipment Specialist' at intervals as recommended by the manufacturer? (usually annually)

  • Are sheave blocks marked with the WLL, manufacturers identification code, nominal size of rope, rope material, rope grade & rope construction?

  • Is the groove of the sheaves correct for the rope size?

  • Are there any other risks or issues associated with the activities observed?
