Information for OSB task

  • What is the issue outside boundary?

  • Were you given accurate as-builts/layplans?

  • DO NOT leave site, first follow the following:
    * Call scoper and request detailed plans.
    * If scoper cannot help, call james Green/Technical support
    * If James cannot help, call your CM and request help.
    If CM cannot help, request referral number and refer to James Green/UFB/Technical support for more information

  • Did you dig exactly as per plans measurements?Did you double check measurements as per plans?

  • Add photos of location you dug up to look for drop-offs as per plans

  • Are you able to install new drop-off?

  • What is required to install new drop-off? (e.g. dig up concrete 3x3m, cut into 26-way and reinstate concrete etc.)

  • Request approval number from CM. Go ahead and install new drop-off. Ensure you draw up as-built/location with measurements of your drop-off you installed. Update notes and rearrangements with tube numbers etc.

  • Did you push-rod from both ends?

  • What surface type needs to be dug up for fix?

  • You MUST rod from both ends to determine possible location of duct break/blockage. Before you refer rod from both ends.

  • How far were you able to push the rod from Boundary side? Measurement?

  • How far were you able to rod from Manhole/Pit/ABFAT? Measurement?

  • Where is the potential blockage/duct break? (Location from measurements from both ends)

  • Did you Blow fibre from both ends?

  • What surface type needs to be dug up for fix?

  • How far were you able to blow from FTP? What is the Measurement?

  • How far were you able to blow from Cabinet/ABFAT/FAT? What is the measurement?

  • If you did not blow from both ends, you MUST blow from both ends and provide measurement from both sides in order to ensure we can locate where the issue is.

  • What surface type needs to be dug up for fix?

  • Certain orders in Year 4 build requires RAT installs (Rapid Access Terminals) - the COE team/scopers/ CM is able to confirm this. This is identified when your allocated FAT in the service order is not at the allocated location as per the Service order.
