House Keeping / General Access

  • Are any walkways, accesses or workplaces obstructed with rubbish, leads, equipment or material ?

  • Are there any openings left uncovered or unguarded ?

  • Are there any timbers lying around with nails left in ?

  • Are there any materials stacked/stored unsafely ?

  • Can you see any overloading to scaffolds, edges of excavations, floor-slabs etc ?

  • Are there any gangways, staircases or work areas with insufficient lighting ?

  • Are welfare facilities provided, with sufficient equipment for operatives, i.e. male / female toilets, with hand washing and drying facilities; canteens with suitable facilities for rest, preparing hot drinks and food; all facilities well maintained, clean, tidy, suitably lit, heated and ventilated?

  • Are there any places where people are working which do not have safe access ?

  • Are there edges that can be accessed which are not properly barriered off ?

  • Can people access dangerous areas? E.g. beneath overhead work or to confined spaces ?

Access To Heights

  • Is the most appropriate means of access being used, e.g. collective protection, level platform, etc, with ladders being a last resort ? <br>

  • Are any ladders - Too short ? - Untied or not footed ? - Defective ? - Being used unsafely ?

  • Are mobile towers wrongly erected or used incorrectly ? (I.e. un-braked or outriggers not set ?)

  • Are any MEWP operators not in possession of appropriate cards / certificates ?

Scaffolding

  • Are there any scaffold boards, toe-boards, rails, and brick-guards missing from working platforms ?

  • Are any boards poorly supported or restrained (i.e. forming 'traps' over-sailing or over-spanning)

  • Are there any gaps between the scaffold and the structure through which persons can fall ?

  • Are there any gaps in the working platform through which bolts, tools or brickbats etc. could fall ?

  • Are barriers or warning signs missing from incomplete scaffolds ?

Public Interfaces

  • Are the public at any immediate risk from current operations on site ? e.g. falling materials ?

  • Is segregation of the public inadequate or poorly maintained? e.g. poor barriers, signage, lighting ?

  • Could the condition of roads or pavement pose a trip hazard or inconvenience any of the public ?

Plant And Equipment

  • Is plant operated or parked where it can fall into adjacent excavations or over edges ?

  • Is plant being driven too fast, with unsecured loads or with illegal passengers ?

  • Is any plant (including hoists and small plant) being operated unsafely ?

  • Do cranes appear to be set up incorrectly, particularly the outriggers and pads ?

  • Are cranes being operated without completed lifting assessment forms ?

  • Are Slinger/Signallers disregarding approved methods of slinging ?

  • Does plant appear in poor condition or badly maintained (oil leaks, noisy, rust, excessive exhaust ?)

  • Are there any moving parts of plant not suitably protected ?

  • Is there a lack of suitable equipment immediately available to support excavations you are digging ?

PPE (Personal Protective Equipment)

  • Are any persons on site not wearing the eye protection required for the task, if indicated by assessment ?

  • Are any persons on site not wearing gloves (if indicated by assessment) or safety footwear with toe and mid sole protection ?

  • Are any persons in noisy areas not wearing ear protection or in dusty conditions without masks, if indicated by assessment ?

  • Are any persons not wearing appropriate clothing or equipment e.g. overalls, high viz, harnesses ?

Site Services

  • Can you see any 240V equipment being used on site ?

  • Can you see damaged electrical connections/fittings or vulnerable electric cables ?

  • Is there a lack of identification or protection to overhead or underground services ?

Fire

  • Do any combustible materials pose a significant fire risk? e.g. LPG, paints, timber etc.

  • Is there a lack of suitable fire points or are they obstructed ?

  • Are hot works procedures being disregarded (including requirements for permits ?)

  • Are site smoking rules being disregarded ?

Materials

  • Are hazardous materials being used with disregard to the instructions on the labels ?

  • Should manual handling be done mechanically ? (if greater than 20kg per person)
