PDB Weekly Inspection Checklist

Are all environmental controls in place?

Does the area have adequate designated parking for equipment and light duty pickups?

Does the task/workers have a detailed JHA and work plan?

DVI's completed for equipment?

Is the housekeeping (garbage, cleanliness, electrical cords, ladders) in good condition?

Is drinking water, sanitation supplies, first aid kits available?

Electrical - proper grounding, LOTO, GFCI, cords in good condition?

All Personal protective equipment being worn?

All accesses (walking/working) surfaces free of ankle biters? Does the access in area have proper signage and delineation?

Electrical tools have their guards in place, are in good condition and are stored properly?

Cranes - rigging equipment such as slings and shackles are properly stored and inspected?

Excavation - properly sloped or shored, permits in place, barricades are up?

Flammables/combustibles stored properly, fire extinguishers, welding and cutting equipment all in place?

Hot work - (PPE, permit, combustibles, flammables) all protected?

MSDS's on site with containers labeled?

Scaffold system fully assembled, tagged properly, inspections complete, fully planked with toe guards?

All barricading/warning signs in areas such as trenches, fuel farms and storage areas all in place?

Fire extinguishers - monthly inspection, accessible on equipment?
