A: ACCESS TO THE CONTAINER TERMINAL.

Is there clearly instruction/ signs instruction the driver to remain in the their truck during loading and unloading operations?

Is the site layout clearly indicates the location of the emergency assembly points?

Is there any dedicated shuttle bus for dispatching the personnel through the terminal including traffic panels and time table?

Sites should be designed so that none of the buildings requiring pedestrian access are located within the operations areas. What about this site?

Limit access for customs clearance to one customer / clearing agent per consignment. Is this practically done ?

All visitors, contractors and subcontractors should receive an access pass from the security entrance point before entering into the terminal. Is this practically done?

Double speed bumps must be in place on each side of the in /out gate. Are they available?

The speed limit in the terminal is it declared for all vehicles that is 20Km/ hr?

All operational terminal staff, contractors and all terminal uses are they instructed that loitering around the terminal is strictly prohibited? All idle staff must proceed to the appropriate waiting areas and cabins within the safety zones.
B: OPERATIONS

High visibility ground markings as well as safety signage should be used to indicate pedestrian crossing. Is it implemented?

Gate traffic/ trucks lanes segregated with barriers/ fences from one another to prevent pedestrian crossing them. Is this done?

Where gate booth in not in place at the entry/ exit gates, creeks must be segregated and protected from the truck traffic lane with installed barriers. Is there booth installed?

Clerks at entering / exit gates on terminal must operate from within barricaded booths in location that allowed truck divers to conduct translations without having to get out of their cabin. Is this practical?

While pedestrian are inspecting containers or cargo, mobile equipment must be prevented from entering and while the equipment is in the inspection area, pedestrian should similarly be prevented from entering. Is this practically done?

If customs physical inspection can not be performed at a designated area by any succumb-stances. Single container inspection area should be restricted by a facing off/ roped off during the examination, is this done?

Customs physical inspection work are they Carried out In areas isolated from yard operations? And should be accessible by protecting the pedestrian walkways to allow customs, clearing agents, customers and security walking to it without being exposed to machine traffic.

Shift roster patterns are they structured to ensure that all equipment operators receive a minimum of break period between shifts or as otherwise required by local legislation?

D: EQUIPMENT MANAGEMENT.

Terminal equipments', contractors' and or visitors' are they fitted with beacon lights?

Reefer technicians can they be able to access 2nd and 3rd tier containers with protected fixed staircase and platform?

Reefer technicians working in the reefer rack/ tower are they considering at all times the minimum safe distance of 5 meters away from working handling equipments?

Machine and vehicles are the inspected daily by the operators and get the inspection signed by the supervisors?

All lifting equipments do they have large high visibility contrast striping applied to their rear counterweight area?

All equipments have reflective striping installed on their sides?

All Mobile equipments are they mounted with reverse alarms?

There is a culture of using seat belts on mobile equipments including light vehicles?

H: WORK PERMIT AND AUTHORIZATION.

An operational toolbox meetings are they conducted daily with the key operational manager? Authorization or cancellations of works are declining during the meeting?

I: TRAFFIC CONTROL.

Radios should be available between the security company, QHSE, Technical and operations department.

Is there any restrictions signs for the use of mobile phones within operations zone?

Pedestrian in authorized areas must be isolated from the movement of all vehicles by the use of barriers and if possible Manually triggered booms.

Is there enough safety signs indicating speed limits (20Km/hr)? Is very important to have those signage at the main entrances and within the yard.
J: HEALTH

Is there any medical point on terminal with a qualified medical person?

Is there any practice in place to conduct random drug and alcohol testing to staff and other people accessing the terminal?

If NO dedicated ambulance in terminal, a contracted ambulance should be made available?

First aid kits are available in designated areas and in all operational vehicles in the yard?

Is there any test conducted for ambulance to observe the response time and reliability?
K: MANAGEMENT.
