  • This planned inspection is applicable to all operations where drilling and blasting is undertaken.
    The purpose of this inspection is to ensure compliance with the requirements of:
    • DM-OC-(OC)-S3 Drill and Blast and its supporting procedures.
    Any magazines are to be inspected using DM-OC-(OC)-F312 Planned Inspection – Magazine and Security Risk Substances

  • Forward a copy of the report to:
    • Safety Manager/Safety Superintendent
    • All persons named in the action plan

  • The Planned Inspection will be conducted by a Supervisor and / or Safety and Health Representative and an employee from the area inspected, and will be submitted to the Area Superintendent / Manager for review.
    Where practicable, all Planned Inspections will include completing a Task Observation or SBO on any activities being undertaken at time of inspection.
    Non-conformances identified during this inspection must be entered into InControl. The person conducting this inspection is responsible for identifying that all non-conformances have been corrected and closed out in InControl.

Drilling Area

  • Is the action to the drill area appropriately maintained?

  • Was the bench inspected by the Driller?

  • Were the wall conditions inspected by the Driller?

  • Is the drill pattern delineated correctly?

  • Do all pit edges have adequate windrows in place?

  • Separation distances from other activities in place?

  • Does the driller have the current drill pattern?

  • Is the marking of hole positions sufficient?

  • Is there sufficient pattern marked in advance?

  • Are appropriate dust controls in use?

  • Are holes plugged when required?

  • Is the driller dipping the holes on completion?

  • Are redrills appropriately marked in the field?

  • Does the Driller know the Lost GET procedure?

  • Is housekeeping adequate?

Blasting area

  • Is the access to the blast area appropriately maintained?

  • Was the bench inspected by the Shotfirer?

  • Were the wall conditions inspected by the Shotfirer?

  • Is the Explosives Loading Area delineated correctly?

  • Do all pit edges have adequate windrows in place?

  • Separation distances from other activities in place?

  • Is housekeeping adequate?

Transport and Storage on Bench

  • Explosives and Detonators transported/stored separately?

  • Transported in appropriate secure containers?

  • Only enough explosives on bench for immediate use?

  • Are quality control checks on explosives completed?

Loading

  • Does the Blast Crew have the current Load plan?

  • Is the current plan being followed?

  • Is explosives usage per hole being recorded?

  • Are only the holes to be fired being loaded?

  • Is the Blast Crew dipping the holes prior to loading?

Firing

  • Are the Blast Notice Boards current and correct?

  • Has a Pre-Blast Review been completed?

  • Has the Blast Control Plan been completed?

  • Have all accesses been identified and guarded?

  • Is an audible signal sounded before and during firing?

  • Is the shot inspected before work recommences?

  • Are all misfires reported?

  • Is a Post-Blast Review undertaken?

Magazine

  • Has the Planned Inspection for Magazines and Security Risk Substances (DM-OC-(OC)-F312) been completed?
