Host WHS Management Systems

  • All questions should be be answered with YES, NO or N/A, all YES answers will require a copy or photo of the document as evidence for the safety team to assess compliance.

  • Is the host accredited to a certified safety management system? (AS 4801 or OHSAS 18001)

  • Does the host have an up to date, signed WHS policy?

  • Does the host have a hazard identification and risk assessment procedures?

  • Does the host have written safe work procedures? (JSA, SOP, SWMS or Work Permits)

  • Does the host have a documented induction process for new workers?

  • Does the host ensure that incidents are reported and investigated?

  • Does the host have emergency evacuation procedures that reflect the type of emergencies they could encounter?

  • Does the host have adequate amenities for the workers?

  • Does the host have safety signage in place?

  • Is housekeeping to a good standard?

  • Does plant and equipment have adequate guarding and emergency stops in place?

  • Take a photo of the plant or equipment.

Verification Statement

  • This document is verification that a WHS site visit has been completed and the AiGTS Employment & Training Consultant conducting the assessment is satisfied that all the information is true & correct.
    Apprentices/Trainees can not start at the host organisation site until this assessment is approved and signed by the safety team and returned to the ETC.
