Please review the list of known generic site hazards and indicate if they are present on this job or not. If they are present you must review the risk controls and ensure that they are complied with once work begins.
Generic Site Significant Hazards
-
Manual handling & lifting. Ensure correct procedure is followed. Get help.
-
Slips, trips and falls. Housekeeping issue. Don't leave things lying around. Clean up after yourself.

Noise and eye damage. Wear hearing protection and safety glasses where necessary.
-
Alcohol and drug use. No persons under the influence of drugs or alcohol are permitted in the workplace. No drugs or alcohol in the workplace.

Stress and fatigue. If stress or fatigue becomes an issue, report it at the outset.
-
Medical conditions and allergies. Advice management of any allergies (i.e. bee stings) and keep medication close at hand.

Dehydration. Drinking water is provided at each site. Use it.
-
Sun exposure. Use sunscreen provided.
-
Training. Ensure staff are fully trained for their job and are supervised where necessary.
-
Repetitive strain injury (RSI). Report any signs at the outset. Don't try to work through it. Take regular short rest breaks and try to diversify work tasks.

Vehicles. Ensure registration and warrants are current. Drive safely. Report any faults or repairs needed.
-
Vehicle movement. Don't assume the operator knows you are there.
-
Diggers and heavy machinery. Stay clear when in use. Don't assume the operator knows you are there.
-
Electricity. Ensure plugs and leads are in good condition. Use safety cables or an RCD.
-
Overhead hazards - wires. Keep a safe working distance of at least 4 metres.
-
Tools and equipment. Includes grinders, welders & power tools - report any faulty equipment. Ensure guarding is in place. Isolate before attempting repairs or maintenance . If unsure ask.

Working at heights. Secure ladders. Take into account wind and weather conditions. Wear all protective equipment necessary.

Mobile Elevating Work Platform. Trained operators only. Use on firm and level ground. Set the outriggers correctly before using the EWP or raising the platform. Make sure the work platform has effective guard rails and tow boards if not fully enclosed. Only use within the manufacturers specified wind rating. Do not park or store the EWP with the platform raised. Use barriers to keep traffic and pedestrians away from where the EWP is operating. Wear appropriate PPE.

Scaffold - Ensure all scaffolding is erected by a certified scaffolder. Don't use scaffolding for storage.

Working on Roofs - Edge protection to be in place. No work on roofs when wet and slippery or if the weather conditions make it unsafe to do so.

Falling Objects - No work underneath anyone working at heights.

Open Trenches - Follow the code of practice for excavations at all times. This includes no work in trenches deeper than 1.5 metres unless they are benched, shored or battered back.

Hazardous Materials - Acids and wet cement - Wear protective equipment and follow the recommended handling instructions on the label or MSDS. Store securely.

Powder Tools - Accredited operators only. Wear protective equipment.

Chainsaws - Follow safety procedures. Keep chainsaws well maintained and chains sharp. Check the saw before use and make sure the chain brake is working. No drop starts. Wear all protective equipment. Do not use the tip of the cutter bar when cutting to avoid any possibility of kickback.

Ripple Plants - No entry without authorization and supervision. Where ladders are used they are to be non-conductive. Keep keys in separate locations. Wear appropriate personal protective equipment. Trained operators only. Be aware of arcing electrical currents.

MSU High Voltage - MSUs are to be caged and signage in place. Be aware that there may be residual current and maintain minimum safe approach distances. Do not touch any equipment you are not authorized to. Trained operators only.

Switch Gear - Take care not to expose equipment to vibration.

LV Panels - Maintain minimum approach distances.

Underground Services - Obtain cable plans and service maps. Cable locate. Dig by hand or hydrovac if unsure or in close proximity. Contact Orion for permits and/or standover.
