  • This Site Safety Inspection forms part of the Fortec Weekly Inspection requirements for Post Tensioning works to requires all works to comply with statutory and regulatory authorities.

Supervisor Assessment

  • Are the Daily Pre-starts and Daily Site Diary entries up to date. Are weekly Tool Box Talks being conducted?

  • Are all employees aware of the Emergency Procedures and know where fire equipment is placed?

  • Are Employees aware of the location where First Aid facilities are placed?

HOUSEKEEPING

  • Are walkways and work areas clear and free of trip hazards?

  • Are safety signs in place and located in the appropriate positions?

FALL PROTECTION

  • Is there perimeter protection/handrails in place?

  • Are all penetrations covered or isolated?

PPE

  • Are long shirts, long pants, gloves, glasses and other required PPE being worn correctly and is in good condition?

PLANT & EQUIPMENT

  • Is plant & equipment maintained and used correctly and records maintained? (Pre-starts, maintenance records etc)

  • Is electrical equipment tested and tagged?

  • Are "Out of Service" tags available for use on site?

  • Are electrical leads suspended?

  • Is guarding in place and adequate to prevent injury? (i.e. can a body part penetrate through the guarding and cause injury?)

  • Are ONLY 7" approved Fein grinders in use? Have all operators of Grinder equipment completed their VOC? Is double eye protection worn by the operator?

ENVIRONMENT

  • Are hazardous substances used and stored according to their SDS?

  • Is there adequate placement of bins and is rubbish being disposed of correctly?

  • Confirm that there have been no hazardous spills on site?

QUALITY ITEM AND PROCESSES

  • Are PCF's being completed for each stage of the works and submitted to the client?

  • Is ALL equipment calibrated and certificates current?

  • Are Strand Test certificates visible on open coils? The technician is to ensure that a coil in use has the test certificate attached to the Bripac at ALL times.

  • Note any improvements you consider necessary. Use this space for additional comments.
