Is equipment sited in a suitable location that is easily accessible for the operator?

Are all operatives competent to maintain and/or operate this machinery?

Are dangerous parts suitably guarded and are guards undamaged?

Any trip hazards caused by trailing leads etc?

Are all parts of the machine safely accessible?

Will operatives be exposed to hazardous substances?

Are all persons protected against contact with hot parts of equipment?

Is isolator accessible and identified and can it be locked off?

Is there a purging board in the vicinity and is it suitably stocked?

Are emergency stops accessible and undamaged

Are electrical controls, plugs and wiring visually undamaged?

If machine poses a fire risk (e.g. naked flame) are suitable protective measures in place?

Are all controls easily identifiable and visible?

Is the provision of lighting around machine suitable and adequate?

Any ladders and platforms that are fixed to the machine in good condition and secure?

Are hopper ladder rings fitted with yellow/black protective rubber foam?

Any other safety related issues with the machine?

Is the machine in its current state Safe to run?
