1. General Induction - The workplace is to ensure that the above named contractor/s have been provided with the following information and/or instructions:

  • Contractor Log Book / Sign In

  • Security and access arrangements

  • Break Glass

  • Emergency Procedures

  • Site occupational health and safety

  • Traffic management plan

  • Hazardous substances and dangerous goods stored onsite

  • Hazard and incident reporting procedures

  • Location of first aid facilities and amenities

  • Required conduct/behaviour (eg. No smoking, offensive language, loud music etc)

2 - Information to be provided by the contractor

  • Licence and qualification details

  • Safe Work Method Statements (SWMS) or equivalent (e.g. Job Safety Aanalysis) provided for all works performed

  • A copy of the contractors current public liability insurance has been provided for records? NOTE 10 million dollars minimum cover required.

  • A copy of the current workers compensation insurance has been provided?

  • Permits to work for High Risk? (eg. high spaces, hot work, confined spaces, etc)

  • I have been inducted in the above information and will comply with the safety instructions listed in my SWMS (or equivalent).
