5) Was the fire alarm heard in all areas?
Was the fire alarm heard from all parts of the building, including toilets and mess rooms?
6) Was the fire alarm signal received at Fire Control?
Did the fire control identify the area of the fire alarm within the building? Did the fire alarm panel function correctly?
7) Was the action of the Fire marshal(s) satisfactory?
Did the nominated fire marshal(s) perform their duties correctly? To encourage occupants to being to evacuate on hearing the alarm. Direct people to safest emergency exit To perform a sweep of a designated area where safe to do so to ensure the area is evacuated To undertake a roll call of occupants To liaise with senior fire marshal or fire and security personnel
8) Was a roll call undertaken in a timely and organised way?
Did the roll call appear organised? Was the roll call list up to date and with correct information? How did staff react to the roll call? Could the person completing the roll call be heard?
9) Overall, was the evacuation completed statisfactorily?
UNSATISFACTORY; The time to evacuate exceeding 3 mins All occupants appear to use the main entrance, rather than their nearest exit Occupants movements are slow, people move against the flow of traffic, collect belongings etc, Fire marshal appear unaware of their responsibilities, do not carry out duties correctly, appear to hinder evacuation The roll call is unorganised, no apparent control or senior fire marshal takes control.
SATISFACTORY; A timely evacuation of all occupants (estimated 2.5mins) Occupants use their nearest available exits rather than the main entrance/ exit Occupants react to the fire alarm and move toward an exit. Occupants move directly toward the assembly point Fire marshal aid the evacuation and perform their duties A roll call is carried out in a simplified and organised way.
