Field Operation Audit

  • Is there a current “Site & Equipment Risk Assessment” available? <br>(Should be a copy with the log card)

  • Is the lift Log Card up to date and completed acceptably?

  • Does the engineer have “Risk Assessment & Method Statement” for the work they are carrying out?

  • Has the engineer been made aware of any site hazards and protective measures or of any exclusion areas?

  • Are there any areas where the engineer suspects asbestos may be present?

  • Is the engineer’s tools and equipment in acceptable condition?

  • Has the engineer’s electrical equipment (anything with a main plug – chargers, vacuum cleaner, power tools) been PAT tested within the last 12 months?

  • Does the engineer have a LOTO kit, Shorting Kit, Landing Doorstop, Electricity test meter?

  • (If applicable) Lifting equipment & ancillary parts – Must be certified within the last 6 months. If hired they need to check and obtain a copy of the test records.

  • Does the engineer have the following safety equipment? Hi Vis vest; Safety helmet; Bump cap, Safety glasses; Goggles; Ear defenders; 1st aid kit; Palm coated; Nitrile; Cut resistant and Rigger gloves;

  • Is the PPE in satisfactory condition? (If not, replacements need ordering).

  • Is the appropriate PPE being used for the work they are carrying out?<br>(Ask them what hazards are present and how they avoid injury)

  • Is suitable and safe access equipment provided by the client where needed / readily available? (Ladders and Barriers).

  • Are they in acceptable condition and being used where necessary?

  • Ask the engineer to demonstrate the safe procedures for accessing the hoistway i.e. accessing the car top and pit? (Describe as they demonstrate).

  • Can the engineer demonstrate how they maintain full control of the lift and lift system when working on the lift? (Prevent unintended movement).

  • Can the engineer demonstrate the knowledge required for controlling electrical and mechanical hazardous / stored energy?

  • (If applicable). Can the engineer demonstrate the knowledge required for hoisting and rigging and supporting a load or lift car safely?

  • Ask the engineer to describe when they would wear a Harness and lanyard?

  • Does the engineer have a harness and lanyard?

  • Is it in a satisfactory condition / any inspection records?

  • Is their vehicle in good order/condition? (Serviced, Tyres not excessively worn, No broken lights or indicators, Tidy inside).

  • Are the materials, parts, tools, substances and other equipment being stored safely and correctly in their vehicle?

  • Does the engineer have a copy of their Engineers Reference Book readily available? (Contains RAMS, COSHH assessments and guidance information).

  • Does the engineer have copies of Waste Transit Notes to record waste transfered from site? (Legal Waste Duty of Care requirement)

  • Is the quality of work completed by the engineer acceptable?

  • Are there any other hazards of concern on site?
