Housekeeping

  • Are work areas clean and orderly?

  • Are employee food, drinks, cups, etc. properly stored and not left randomly laying around?

  • Are all materials and equipment stored in a way that prevents a hazard to employees as they work or walk by?

  • Are materials properly stored with no leaning items and not stacks greater than 4' tall?

  • Are floors absent of slip/trip/fall hazards (oil, water, grease, cords, hoses, debris)?

Personal Protective Equipment (PPE)

  • Is proper eye protection being used?

  • Is proper hearing protection being used when and where needed?

  • Are eyewash stations available and are they accessible, clean, and in-date?

  • Are employees that paint using respirators and have they been medically evaluated and fit tested?

  • Are respirators clean and stored properly to prevent contamination to the user?

  • Are employees wearing gloves when handling metal and/or hot items?

  • Are employees wearing proper protective footwear (toe protection)?

  • Is PPE readily available for all personnel including visitors?

Postings/Administrative/Training

  • Are OSHA 300 Logs posted (between Feb. 1 thru April 30 each year?

  • Have employees been trained on emergency and evacuation procedures?

  • Are fully supplied first aid kits readily available, easily accessible, and stocked with in-date supplies?

  • Are PPE Required reminder signs posted at all shop entrances?

  • Are there safety signs posted in the area?

  • Are the safety requirements posted on each machine?

  • Are Lockout/Tagout procedures posted on each machine?

Walkways, Aisles, and Exits

  • Are all exits and passage ways kept clear?

  • Are all exits marked with an illuminated exit sign?

  • Are all non-exit doors marked accordingly?

  • Are holes or damaged walkways properly covered, marked, or otherwise made safe?

  • Are exit doors left unlocked during operational hours?

  • Are exit doors designed to be opened in the direction of travel?

  • Are standard guardrails provided wherever aisles or walkway surfaces are elevated more than 48" above any adjacent floor or the ground?

  • Are standard 4" high toeboards installed around the edges of permanent floor openings where persons may pass below the opening?

  • Are all steps slip resistant?

Portable Ladders

  • Are all unused ladders secured to a wall by a hook or other means of securement?

  • Are ladders maintained in good condition?

  • Are ladders free of unauthorized repairs or modifications?

  • If the ladder is used in front of a door or exit, is the exit blocked from use while the ladder is being used?

  • Are ladders inspected prior to each use and are monthly inspections documented?

Lockout/Tagout (LOTO)

  • Is all equipment capable of being locked out as needed?

  • Are all affected employees trained on LOTO?

  • Are all authorized employees properly trained on LOTO?

  • Does the authorized employee check the safety of the lockout by attempting a startup after making sure no one is exposed?

  • Are all authorized employees that are exposed to dangers properly locking out with each of their own locks?

  • Are authorized employees required to keep personal control of their key to their personal lock?

Fire Protection

  • Are fire extinguisher monthly inspection dates current?

  • Are fire extinguishers mounted within 75 feet of each other and stored in their designated places?

  • Are fire extinguishers serviced on an annual basis?

  • Are fire extinguishers readily accessible and not blocked?

Emergency/Evacuation Plans

  • Do employees know what to do in the event of a fire or tornado?

  • Are signs and maps posted for designated evacuation routes, areas, and are evacuation routes kept clear?

  • Are annual plant evacuation drills conducted and documented?

Hand Tools, Power Tools, and Equipment

  • Do tools such as grinders, saws, and similar equipment have proper guards and shields in place?

  • Are power tools properly grounded or double insulated?

  • Are chisels and punches free of mushroomed heads?

  • Are there effective guards in place over belts, pulleys, chains, and sprockets on all equipment within 7' of the floor or working level?

  • Are portable or stationary fans provided with full guards or screens having openings of 1/2" or less?

  • Is the bench grinder tool rest and tongue guards adjusted properly (work rest 1/8" from wheel, tongue guard 1/4" from wheel)?

  • Are all tools and machines kept clean and properly maintained?

Welding/Compressed Gases

  • Are welding gas cylinders properly secured?

  • Are compressed gas cylinders stored in areas protected from external heat sources?

  • Are fuel gas cylinders and oxygen cylinders separated by at least 20' or fire resistant barriers at least 5' high while in storage?

  • Are empty and full cylinders identified accordingly?

  • Are cylinders marked to identify the type of gas?

  • Are dollies being used to move cylinders greater than 10'?

  • Are all valves closed and capped before a cylinder is moved?

Compressed Air

  • Are compressors and storage tanks equipped with functioning pressure relieve valves and pressure gauges?

  • Are monthly pressure relief valve inspections documented?

  • Are employees using compressed air properly?

  • Are air nozzles free of damage or unauthorized modifications?

  • Are air nozzles equipped with a means of pressure relief so that no more than 30 psi can be achieved at the tip?

Powered Industrial Trucks (PIT)

  • Are employees properly trained in the use of the type of industrial truck they operate?

  • Does each PIT have a warning horn, whistle, or other device that can be clearly heard above normal noise in the areas where it is operated?

  • Do all employees where seatbelts when operating the PIT?

  • Are all daily inspections being conducted and recorded?

  • Are dock plates and loading ramps in good repair?

Hazard Communication (HAZCOM)

  • Are SDS books readily available to employees?

  • Have employees been trained on HAZCOM and the locations for the SDS books?

Flammable Materials

  • Are all flammable liquids kept in closed containers when not in use?

  • Are all flammable/combustible liquids returned to approved flammable liquid storage cabinets at the end of work operations?

Electrical

  • Do all extension cords have a grounding plug?

  • Are exposed wiring and cords with frayed or deteriorated insulation repaired or replaced promptly (not with electrical tape)?

  • Are flexible cords or cables free of splices and electrical tape?

  • Are all disconnecting switches and circuit breakers labeled to indicate their use or equipment served?

  • Are electrical enclosures such as switches, receptacles, junction boxes, etc. provided with tight fitting covers or plates?

  • Is all electrical wiring protected with conduit and free from exposed wires?

  • Are all unused openings in electrical enclosures and fittings closed wit appropriate covers, plugs, or plates?

  • Are all breaker panels and disconnects free from obstruction and easily accessible?

  • Are transformers and electrical panels not being used as storage tables?

General

  • Are restrooms kept clean and sanitary (SMS and Kiosk)?
