Instructions

  • 1. Inspections are to be conducted every 3 months preferably by a different employee each time (schedule on intranet).

  • 2. Read each checklist item on the inspection checklist, selecting either ‘YES’, ‘NO’ or ‘N/A’ for each one.

  • 3. When ‘NO’ is selected for checklist item, the matrix is used to determine and record the risk level, the required corrective actions, person responsible, due date and (if implemented) date completed.

  • 4. Complete in consultation with your HSR and with your supervisor, Coordinator or Manager (sign on last page).

  • 5. Completed checklists must be uploaded to the WHS Submitted Forms Register on the intranet. The WHS Unit will add corrective actions to the Corrective Actions Register and actioned staff will be notified. MERITS do not need to be entered as corrective actions.

  • 6. Discuss any issues, actions and outcomes in team meetings and at SAG meetings.
