  • The purpose of today's visit is to undertake a statutory scaffold inspection in persuance of sections 12 & 13 of the W.A.H. Regulations 2005. Further guidance documents / regulations refered to in this report include : The H.S.W.A. 1974, SG4:15 ( Preventing falls in scaffolding), TG20 : 13 & any system scaffolding guidance documentation that should be available on site (if system scaffolds are being used).

Previous Scaffold Inspection Close Outs.

  • Have the Failed Responses from the previous Scaffold Inspection been closed out by the Site Team?

Design Drawings/ Scaffold Compliance Sheets

  • Are Designs Available for structures that are non standard?

  • Are Scaffold Compliance Sheets available on site for Standard Scaffold Structures (as per TG20:13)?

  • Have kentlidge bays been loaded out with adequate materials as per the scaffold design?

  • Has the scaffold been erected in accordance with the design or scaffold compliance sheet provided?

System Scaffolds

  • Is there a copy of The Manufacturers Instruction Manual (M.I.M.S.) available on site?

  • Has the System Scaffold been erected in accordance to the M.I.M.S. ?

  • Is there a need for Design Input for the System Scaffold? If so are Design Drawings available?

  • If Loading Bay Gates are Side On is there a Design in place for this?

  • Are all loading bays fitted with up and over loading bay gates? Are are loading bay gates fitted with maximum loading signage?

  • Are Loading Bay Gates Operating as Intended (open and close freely)?

  • Are Loading Bay Gates Closed once a load has been deposited?

  • Are scaff tags in position and each access point?

Scaffold Handover Certificates.

  • Are Scaffold Handover Certificates held on site?

Footings, Sole Boards & Sole Plates.

  • Are the footings provided by the client Suitable for their intended purpose?

  • Is the ground suitable to take the imposed loads of the scaffold?

  • Are Sole Boards of a Suitable Size (no less than 470mm in length)?

  • Have Sole Plates been fitted under all standards?

Standards & Legers.

  • Are all the standards and Ledgers correctly plum or level within the allowed tolerance?

  • Are all joints staggered?

  • Are the lift heights and bay sizes compliant with table 1 of TG20?

Transoms

  • Are the lengths of all transoms sufficient?

  • Is there a transom fitted within 300mm of the node point at ledger braced pairs of standards?

  • Are the spacings of transforms correct on boarded and non boarded lifts?

Ledger, facade and plan bracing.

  • Is all bracing connected within 300mm of the node point?

  • Is bracing connected at the correct intervals with suitable sized tube?

  • Is all bracing connected via load bearing fittings?

Fittings.

  • Are all fittings in a suitable condition?

Ties.

  • Are all ties connected at the correct intervals and have tie tags where applicable?

  • Have proof pull tests been carried out?

Boarded lift.

  • Are Scaffold boards in good condition?, free from splits & cracks?, free from signs of rot & warping/twisting?

  • Are there excessive materials left on the boarded lift?

  • Are there any gaps or notches in the boards?

Handrails, toe boards & Brick guards.

  • Are handrails and toe boards fitted where there is a risk of persons or materials falling?

  • Are handrails and toe boards joined correctly and at the correct height?

  • Are brick guards in a good condition and suitably secured?

  • Is there a requirement for debris netting or mono flex and is it fitted correctly?

Scaffold Access Options (1. Haki Stairs, 2. External Ladder Tower, 3. Internal Ladder Tower, 4. Pole Ladder (not to extend more than 4.5 metres vertically) scaff tags and signage.

  • Is the means of access suitable according to specification?

  • Are scaff tags located at all access points of the scaffold?

  • Is all the necessary signage on display (i.e. on scaffold loading bay gates, scaffold incomplete signage?)

Rubbish chutes?

  • Are rubbish chutes fitted correctly and at suitable intervals?

Housekeeping.

  • Is there a suitable lay down area and is it in a tidy condition?

  • Are all spare materials in a neat and tidy condition?

  • Are the working platforms being kept clean and tidy?

  • Are materials not being stacked/stored on unfinished walls prior to scaffolds being altered/lifted?

SG4:15

  • Does the scaffold appear to have been erected in accordance with SG4:15?

  • Are all scaffolders wearing safety harnesses & clipping on where necessary (following the guidance contained within SG4:15)?
