Written LOTO PROCEEDURE

  • Written LOTO procedures shall be developed for all equipment and processes not meeting any one of the exceptions listed below:
    1) There is no potential for stored energy or a buildup of new energy after shutdown.
    2) The piece of equipment only has one energy source that can be readily identified and isolated (includes plug and cord). This energy source must be in total control of the worker performing the activity at all times. The source must be locked out using a lock out device if the cord is out of sight or outside direct control.
    3) The lockout of that energy source by one lockout device will completely remove all energy from the machine or equipment.
    4) The equipment is isolated and locked out while the work is being done.
    5) The lockout device is under the sole control of the qualified employee who is performing the work.
    6) No hazards are created for any other employee equipment.
    7) No instances of unexpected activation have occurred on the equipment or the machine while it was being worked on.
