Responsibilities for all aspects of a LOTO to be performed must be agreed upon and documented by all affected parties.
LOTO which involves a utility disconnect will be classified as Complex LOTO (use Complex LOTO Procedure Sheet).
LOTO which requires procedures for LOTO continuity across shifts will be classified as Complex LOTO (use Complex
LOTO Procedure Sheet).
LOTO which requires LOTO by other parties (e.g., customer, subcontractors, other contractors) will be classified as

LOTO CHECKLIST
-
The 1st section of this checklist must be completed,
signed and visibly posted at the point of work
!! BEFORE STARTING WORK!! -
STEP 1: PREPARATION STAGE
- Drawings (single line, layout…) available/up to date?
- Switching schedule (sequence of operations) defined?
- Briefing with customer’s person in charge?
- Appropriate PPE available and checked?
- Correctly rated voltage tester available?
- Adequate earthing (grounding) equipment available?
- Adequate lock out devices and padlocks available?
- Emergency exit & fire extinguishers available?
- Emergency numbers posted - first aid, fire brigades, etc?
- Access to working area restricted with barriers & signs?
- Is SF6 gas present?
-
PREPARATION STAGE (Selected and approved (signed) by SE Person in Charge:
-
STEP 2: REMOVE THE LOAD
- Shut down the load in the normal way
- Local/remote control disconnected
-
REMOVE THE LOAD STAGE (Selected and approved [signed] by the Customer Person in Charge)
-
STEP 3: DE-ENERIZE
- All phases/live conductors disconnected
- Fuses removed
- Feedback from other source: Ring circuits, generators, UPS, batteries, dual fed systems, mesh network, etc
-
DE-ENERGIZE STAGE (Selected and approved [signed] by SE Person in Charge)
-
STEP 4: ISOLATE
-
ISOLATE Stage (Selected and approved [signed] by the SE Person in Charge)
-
STEP 5: VERIFY NO VOLTAGE * Note * All circuits are considered energized until verified.
- Voltage tester rated at appropriate KV, with conformity mark.
- Voltage tester properly tested before use.
- Verification of absence of voltage on all conductors (load and line) in each cabinet.
- Voltage tester properly tested after use (VAV)?
- Was the customer person-in-charge present during voltage verification
-
VERIFY NO VOLTAGE STAGE (Selected and approved [signed] by SE Person in Charge)
-
STEP 6: GROUNDING This is not mandatory for below 600V, unless there is a potential for back feed.
- Ground cables properly connected to ground?
- Ground cables properly connected to ground? Mark equipment as "grounds applied" or place a "ground lock" on the disconnect.
- Capacitors discharged/short-circuited?
- If back feed from other sources is possible, ground cables connected before and after using work area.
-
GROUNDING STAGE (Selected and approved [signed] by Customer Person in Charge)
-
STEP 7 PROTECT CIRCUITS
- Prevent access to adjacent energized circuits (That cannot be de-energized) with insulated sheeting
-
PROTECT CIRCUITS STAGE (Selected and approved [signed] by Customer Person in Charge)
-
After even the shortest interruption of the work process (i.e., restroom, lunch, etc.), perform a visual check to
ensure all controls are in place (locks, tags, barriers, grounding, and short-circuiting when more than 600V). -

STEP 8: ELECTRICAL TESTING
-
ELECTRICAL TESTING STAGE (Selected and approved [signed] by Customer Person in Charge)
-
STEP 9. TEMPORARY RELEASE OF LOTO/REMOVAL OF GROUNDS FOR TESTING
- Temporary release of LOTO required to calibrate a sensor, verify a voltage reading, etc.
- Are hazards present, since it is not possible to replace all guards, doors, etc.?
- Is there a barrier in place to keep everyone outside the hazardous area?
- Is the person who must come in close contact with the hazards fully protected?
- CAUTION! If the equipment must be grounded to be considered de-energized, then it is considered energized if grounds are removed for testing.
-
TEMPORARY RELEASE OF LOTO STAGE (Selected and approved [signed] by SE Person in Charge)
-
STEP 10. RELEASE OF LOTO/REMOVAL OF GROUNDS
- Working area cleared of tools and equipment?
- Covers and doors are replaced?
- Are all persons accounted for?
- All persons without correctly rated Arc Flash PPE outside the arc flash boundary?
- Safety documentation returned?
- A full inspection of all works must be completed by a competent person before re-energizing. (consider all downstream circuits)
- Screening/marking off of working area removed
- Short circuit removed before earthing
- All isolation measures removed (lock, etc)
- Usual safety devices and safety signs re-placed
- Signaling “Ready for switch on”
- … by all work areas
- …by all switching locations
- … by customer
- … by supervisor
- Handing over of modified drawings (if modifications have been done) to operation staff
- Permission to restore energy
