General

  • Has the PICW logged into Netwatch, where required?

  • Has the daily risk assessment (SSWP) been completed and all on site briefed and signed on?

  • Is the contractor in possession of a valid permit to work?

  • Is supervision of safety on site effective and cited out in a competent manner? (Buddy System).

  • Have the SIRO Minimum Electrical Safety Controls (MESCs) relevant to the work activities as a minimum been implemented in the Safe System of Work?

  • Do all people have the correct accreditation to work on site eg, SIRO Safety ID cards, level of training?

  • Is there a record of valid safety training/certification for all staff (Safe Pass, CSCS, MEWP, Power Tools etc).

  • Is there evidence to demonstrate that the work activities have been risk assessed, and incorporated controls into their safe system of work?

  • Scope of work defined (contractor work pack) and evidence of communication to all staff?

  • Can the contractor confirm that the proposed schedule of work has been communicated to the local ESB Network Controller?

  • Is there a copy of the construction phase health and safety plan available for all tasks?

  • Have the working personnel been briefed on relevant safety info including H&S plan.

  • Is the construction lead aware that they are the PSCS representative on site and of their responsibilities?

  • Is there a copy of the contractors safety statement on site? Are they adequate?

  • Are the controls mentioned in the SHP being implemented on site and are they adequate?

  • Is PPE being maintained and being used and worn correctly?

  • Is there a first aider on site?

  • First aid box adequate for works?

  • Is site secure and the appropriate safety signage in use?

  • Is there evidence of good housekeeping on site and are all walkways clear of trip hazards?

  • Is traffic adequately controlled?

  • Is there suitable fire fighting equiptment on site?

  • Are hazardous substances on site used/stored correctly and crew hold a safety data sheet?

Electricity, Overhead Lines, Vaults and Minipillars

  • Are goal posts requires? If yes,bare they adequately constructed?

  • For works in ESB inner substations, has a designated work area been set up?

  • Are crew members aware of the status of the electrical network, the agreed extremities for the work?

  • Is PICW aware of the aware of the limits of proximity?

  • Are earths fitted correctly?

  • Has voltage detector been used ? (mini pillar check).

  • Is red flagging or proximity band required? If required is it installed correctly?

Rigging & Climbing

  • Has a pre work climbing pole & stay check been carried out?

  • Has all climbing equiptment been certified?

  • Is there adequate fall protection in place to prevent falls from height?

  • Are the adequate controls in place to prevent persons being struck in the head by falling objects?

  • Are weather conditions and lighting suitable for tasks to be undertaken?

Excavations & Ducts

  • Are all excavations adequately protected?

  • Is there safe access and egress into the excavations?

  • Are all drawings on site for underground services?

  • Is the excavation shuttered or battered correctly?

  • If cutting into ducts, is the Feinmaster tool, with depth depth restrictors being used?

  • Has a CAT, and/or GPR, been used? Is vacuum or hand dig excavation methods being used?

Plant

  • Is all plant on site in good condition and suitable for the work?

  • Is an up to date register available for all lifting equiptment and lifting tackle?

  • Are preventative measures in place for the protection of members of the public and home owner? 360 check/ for vehicles, banksman for reversing?

  • Is there a lift plan/method statement in place for HIAB/MEWP activity?

  • Is plant earthed correctly where required?

  • Are potable power tools using 110v operated, battery operated or authorised by exception?

  • Are appropriate ladders being used on site?
