Site Information & Procedures

  • Is the correct construction sign visible and completed correctly?

  • Are site specific hazards highlighted on the sign e.g.: overhead power lines, gas lines/open trenches etc.

  • Have trades/suppliers onsite been inducted and are operating within the parameters of their SWMS?

  • Are fall from heights adequately controlled?

  • Are site fall hazards (e.g retaining walls) controlled and identified on site sign or SWMS?

  • Have all persons on site completed the Sienna Homes induction process or signed the sign in book?

Site Conditions/Housekeeping

  • Cross over appropriately installed and maintained?

  • Is the site "trade ready"?

  • Perimeter fencing installed and maintained correctly?

  • Are the driveways , footpaths and naturestrips clear of materials?

Hanging Bracket Scaffold & Working at Heights

  • Scaffold has identified access & egress point to hanging bracket scaffold and roof area?

  • Is the scaffold certificate in place and signed off?

  • Are the mid rails, top rails, kick boards and lap boards off the bracket scaffold in place?

  • Are the scaffold decks clear of materials and debris?

  • Clear access to entry point for ladder?

Void Protection (High Risk)

  • Is the void protection in place with certificate? (To be installed prior to 1st floor walls & flooring)

  • Is the void cover down whilst works occur on the 1st floor?

Roof Guard Rail Condition & Access (High Risk)

  • Is there a barricaded drop zone for roof tiler spoils?

  • Is the area clear of tradespeople under the roof tilers?

  • Has the roof guard rail been installed for roof work?

  • Does the roof guard rail have at least 2 identified access points?

  • Is the balcony guard rail installed? (as required)

  • Are all window openings above 2m high barricaded with timber or rails?

  • Does the balcony/Alfresco have sufficient structural support?

Ladders (High Risk)

  • Ladders in good working condition, fully open and of 1:4 vertical ratio?

  • Ladders secure/tied off/when accessing heights ladders to extend 900mm past roof line?

  • Only Industrial grade ladders to be used (minimum 120kg)

  • Ladders not to be used on balcony's or close to edges unless guards are in place.

Items of Plant & Trenches (High risk)

  • Works requiring licensed operators (excavators, skid steer, forklifts, crane, boom pumps, spotters, riggers/dogmen)

  • Is the pre-start checklist & SWMS completed for the required works and mobile plant?

  • Warning devices operational (reverse beepers, flashing lights & horn?)

  • Is pedestrian traffic allowed for in making the site safe? (Install bollards or temp fence around machine access area to divert pedestrians)

  • Does the plant look fit for purpose or use e.g: Earthmover quick hitches secured/pinned?

  • Are the exposed screw pile holes/piers covered or barricaded to prevent falls?

  • Large exposed trenches have fences/barriers in place? (Trenches in excess of 1.5m deep need to be battered or shored up.)

  • Soil piles to be located a minimum of 1m from edge of trench.

Electrical Tools & Equipment

  • Site meter box RCD has been tested monthly/ main fuse locked and door closed? (Must be tested prior to first use of installation)

  • If a generator is used it should have a RCD fiited and not used indoors or confined space (fumes)

  • Electrical equipment (leads & tools) tested & tagged every 3 months and in date. RCD monthly

  • Are electrical leads exiting through the bottom of the meter box?

  • Are electrical leads raised up where practical and minimum 15amp leads (1.5mm)?

  • Leads to be no longer than 25 metres and in good working condition (no exposed wires and not doubled up)

  • Equipment has required guarding in place (e.g: Drop saw/circular saws)

  • Drop saw and grinders have blade guards in place?

Trades

  • Does the trade have an adequate first aid kit and is first aid trained?

  • Are the trades working sun smart?

  • Is the incident register up to date?

  • Chemicals/materials used by the trade have MSDS?

Personal Protection Equipment (PPE)

  • Appropriate footwear for the task being worm?

  • Sunscreens is available and is being used?

  • Eye protection is being worn as required?

  • Hearing protection is being worn as required?

  • Dust mask is being worn as required?

  • Hat (hard hat) is being worn as required?
