Barricades/Tape/Flagging

  • Are there any barricades/tape/flagging being used to deter people or pedestrians from hazards?

  • Is the type of deterrent being used appropriate for the hazard present?

  • Does the barricade/tape/flagging completely surround the area?

Confined Space Entry

  • Is there any work being conducted in a confined space?

  • Has a confined space entry permit been issued?

  • Was initial air monitoring conducted?

  • Are their atmospheric, configuration or engulfment hazards that would make this a permit required confined space?

  • Is there a documented rescue plan?

  • Is there an attendant that has communication with the worker?

  • Is there a harness/life line in place?

  • Is continuous air monitoring in place?

Mobile Equipment

  • Is there any mobile equipment in use? Forklifts, mobile crane, lull, scissor lift, etc

  • Has a pre shift inspection been completed and documented for each piece of equipment?

  • Are all safety features in place and functional? (lights, alarms, seat belt, etc)

  • Are all loads properly secured?

  • Are operators obeying all traffic rules? Stopping at stop signs, communicating with pedestrians, obeying speed limits, etc

  • Are spotters being used where needed?

  • If the equipment is unattended is the ignition off and attachments lowered to their safe resting position with the brake set and controls in neutral?

Aerial Lift

  • Are there any aerial lifts in use? (including scissor lifts)

  • Are feet on the lift floor? (employees should not have feet on the mid rail or outside of the lift)

Cranes

  • Are there any cranes in use?

  • Has a lift plan been completed and reviewed?

  • Is the swing radius protected?

  • Is there a light/flag on the boom tip?

  • Is the load properly rigged?

  • Is there a signal person in place?

  • Are slings/rigging properly stored and have a current inspection?

Material/Equipment Storage

  • All equipment and materials are stored in a way that does not create blind spots?

  • All equipment and materials are stored in a way that does not block or protrude into aisles/roads/walkways?

  • All equipment and materials are stored in a way that makes the work zone look generally neat and organized?

  • All equipment and materials are stored in a way in which they are stable and do not create an overhead or falling hazard?

  • The amount of combustible liquids stored inside does not exceed 600 gallons

Electrical

  • Are employees working near high voltage equipment?

  • Is there at least a 10 ft clearance between employees and any high voltage electrical equipment?

  • Is there at least a 3 ft clearance for all electrical panels and disconnects?

  • Are all electric panel doors closed?

  • Are all panels and disconnects labeled?

Extension Cords

  • Are there any extension cords in use?

  • Are all cords in good condition?

  • Do all cords have either a 3 prong ground or are double insulated?

  • Are all cords secured to the ground or flagged to reduce the risk of a trip hazard?

  • Are all cords protected by GFCIs?

Hot Work

  • Is any hot work (welding,cutting,grinding,brazing) being conducted?

  • Has a hot work permit been issued?

  • Is there a 35 ft area that is clear of combustibles and ignitable liquids?

  • Is there a fire watch in place?

  • Is there a fire extinguisher within 20 ft?

  • Are welding curtains in place?

  • Are welders using appropriate PPE? (cotton shirts, welding helmet, welding gloves, FR pants)

  • Are all welding lead connections protected? Gauges not broken? Flash back arrestors and back flow preventors in place?

  • Are employees holding the unguarded part of the grinder away from their body?

Compressed Gas/Cylinder Storage

  • Are there any compressed gas cylinders on site?

  • Are all cylinders secured and capped if necessary?

  • Are all cylinders properly labeled?

  • Are oxygen cylinders separeated from fuel-gas cylinders and combustibles by at least 20 ft?

PPE

  • Are all employees wearing hard hats?

  • Are all employees wearing safety footwear?

  • Are all employees wearing safety glasses? (tinted only allowed outside)

  • Are all employees wearing a high viz outer layer?

  • If required, are all employees wearing ear protection?

  • Are all employees wearing appropriate gloves for the task they are completing?

  • Are employees free from wearing any jewelry, strings, hoods or potential entanglement hazards?

  • If necessary, are face shields or googles being worn?

  • If necessary, are respirators being worn?

Fire Equipment

  • Is there at least one fire extinguisher on site?

  • Is all fire equipment easily accessible and not blocked and has proper clearance? (fire extinguishers, sprinklers, hose connections)

  • Have extinguishers been inspected in the last month?

  • Do all cranes and vehicles have a fire extinguisher?

Lockout Tagout

  • Is any equipment locked out?

  • Does the lock and tag indicate who the owner of the lock is and the company they work for?

  • Were lockout tagout procedures followed?

  • Are the owner of the lock in control of the key?

Machine Guarding

  • Are all machine guards in place and in good condition?

  • Are all grinder tongue guards set at 1/4" and tool rests at 1/8" openings?

Eyewash

  • Is there an eye wash station available?

  • Is the eye wash station easily accessible?

  • Is the eyewash solution within its usage date?

Containers

  • Are all secondary containers labeled and in good condition?

Fuels/Flammables

  • Are there any fuels/flammables being stored on site?

  • Are drums, totes and buckets used for refueling inside or transferring flammable liquids grounded and bonded?

  • Are flammables stored in appropriate cabinets?

  • Are no smoking signs posted where applicable?

  • Is there a stocked spill fit near the fueling station?

Housekeeping

  • Are all areas (work zone, conex, lay down, yard, material storage area, parking etc) clean and organized?

Start Up

  • Was a JHA completed or reviewed at start up?

  • Was a toolbox talk conducted at start up?

Environmental

  • Is the area free of any spills, chemicals, mud or contaminated water going into floor or storm drains, or entering water sources?

  • Is the area free of any containers stored next to drains?

  • Are any chemicals or oil being disposed of in sink drains?
