2. Fire

3. First Aid

  • First aid kit is adequately stocked (as per the Schedule in the kit)

  • Area clear around first aid kits providing safe access

  • First Aid kit locations posted clearly, workers aware of locations

  • At least one worker on site with current Senior First Aid cert

  • Eyewash stations available and safe to use

4. Electrical

  • No broken plugs, sockets or switches

  • No frayed or damaged leads

  • Portable power tools in good condition

  • All temporary leads up on cable stands

  • All electrical equipment has a current test tag and is in date (3 months Construction equipment, 12 months for Office equipment)

5. Housekeeping

  • Work area neat, debris picked up and free of trip hazards

  • Bins are not overflowing

  • Work benches clear of rubbish and tools stored properly

  • Materials properly stored, clear of stairways and exit routes, and stacked using 4:1 maximum stack height to base ratio

  • Cords/welding leads/hoses off walking/working surfaces

  • Nails removed or bent down

  • Potential impalement hazards from protruding rebar, form stakes, pipe stubs, and nails removed, capped or covered

  • Tools and Bolts on upper levels of buildings stored in buckets and not potential dropped object

  • Work areas are clearly defined and segregated

6. Personal Protective Equipment

  • Hard Hats are being worn on site

  • Safety glasses are being worn and meet AS/NZS 1337 Standard

  • Gloves available and being used for manual tasks

  • Steel capped safety boots are being used on site

  • Dust masks are available for use by all employees

  • Hearing protection available and used when required

7. Excavations

  • All barriers at least one (1) metre high and erected at least two (2) metres from the<br>edge of trenches or excavations

  • Signed and approved permits available for all current excavation activities

  • For excavations exceeding one (1) metre in depth, signs placed at appropriate locations around the perimeter of the area where they are<br>easily seen

  • Trenches or excavations more than one (1) metre deep have access ladders or ramps<br>installed to ensure safe entry and exit at maximum intervals of 30 metres apart.

8. Cranes & Rigging

  • Pre-shift crane inspection completed by a competent person

  • Annual crane inspection (green sticker) conducted, documented and on file

  • HSEQ-6-12-F01 High-Risk Crane Work Permit available for active crane operations on site

  • Crane is clearly labelled with its rated safe working limit (SWL) capacity

  • Licensed operator in control of the crane

  • Licensed rigger in control of the rigging

  • Chains, slings and all lifting gear of a sound standard, tested, tagged and inspected accordingly

  • Adequate Tag lines are used to guide loads (no hands on loads above waist height)

9. Scaffolding

  • Scaffold erected/ dismantled by licenced person

  • Scaffold is inspected and tagged safe for use by a qualified person

  • Platform boards lashed, level and no gaps

  • Access ladders lashed and protrude at least 1 meter past landing

  • Working platforms are complete with Handrails, mid rails and toe boards

  • All penetrations for access have suitable trapdoors

  • All gates are spring loaded and open towards the platform and not the unprotected edge

10. Ladders, Stairs, Platforms and Walkways

Ladders

  • Ladders are of proper type and size for the task (no over reaching/ extending or working off rungs)

  • Platform ladders in use for work below four meters

  • All ladders to have Working Load Limit (WLL) of no less than 120 kilograms

  • Unless a fall arrest system is used, two-legged straight or extension ladders used for access and egress

  • Defective ladders removed from service and tagged

Stairs, Platforms and Walkways

  • Handrails, mid rails and toe boards installed on all stairways, platforms and walkways

  • Anti slip nosing installed on all stair treads and clearly visible against the background

  • Access to areas with incomplete stairs, platforms and walkways has been physically barricaded and access only granted under a high risk permit to persons with the appropriate licences and tickets

11. Chemicals

  • SDS available for all chemicals stored on site

  • Containers are clearly and accurately labelled

  • All chemicals are stored in accordance with the SDS

12. Registers

  • Training Matrix available and current

  • SDS Register available and current

  • Lifting and Rigging gear register available and current

  • Mobile Plant register available and current

  • Electrical Register available and current

  • Project Hazard Register available and current

13. Hot Work

  • Current, signed and authorised permit available for all hot work tasks being conducted on site

  • Charged and in date Fire extinguisher in close proximity to the works

  • Ground below the hot works is clear of all flammable/ combustible material (paper, cardboard ect.)

  • Fire watch/ Spotter available if required in the term of the permit

14. Working @ Heights

  • A current, authorised Working at Height permit is available for all tasks being carried out at height

  • All persons conducting working at heights are adequately harnessed, appropriate for the application of work being completed (fall restraint/ fall arrest, etc...)

  • All tools are attached to lanyards and adequately secured to prevent a dropped object

  • Adequately controlled Exclusion Zones and Drop Zones are in place and take practical consideration for deflection of possible dropping objects

EWP's

  • EWP's have a completed prestart inspection by licenced operator. Any/ all identified faults have been attended to in line with company site procedure.

  • All required classified plant inspections and registrations are current

  • Equipment has been inspected and approved for use on Boral work site

  • EWP is being operated by appropriately licenced operator

  • A dedicated and appropriately licenced spotter is identified and available for all EWP works
