BEFORE STARTING ENGINE
-
Daily Pre-Use Inspection Completed?
-
Walk around skid loader and look for hazards or damage.
-
Document all findings & notify supervisor.
ENTRY AND START-UP
-
Lift arms down, use steps made for entry.
-
Does not use control levers as grab bars.
-
Adjust seat, buckle seat belt, restraining bar lowered (if equipped).
-
Controls in neutral and parking brake set. Check area is clear of bystanders.

Start machine, test all controls, check brakes.
SKID LOADER DRIVING
-
Start machine movement smoothly.
-
Drive to pre- determined point, increasing throttle and speed, maintains good control of the machine.
-
Test brake or traction system (to make sure it will hold)
-
Come to a smooth, steady stop, and avoids obstacles and hazardous areas.
-
Check for clear travel path, backs machine, turning at 90 degrees, comes to a stop, turns back towards the start area.
-
Drive to original start area, reduces throttle and speed when entering the start point.

Tilt Alarm Testing
LOADER BUCKET USE
-
Square up to material to be moved, lower and level the bucket.
-
Drive forward, obtain a load, tip the bucket back to stabilize the load.
-
Lift load smoothly until clear, keep load at low travel height, look over both shoulders and back up.
-
Turn and move forward, maintain safe speed, travel to load destination.
-
Make sure of sufficient clearance to raise the load, raise the load to placement level.
-
Move slowly forward, tip bucket forward to dump load smoothly.
-
Once empty, tip bucket back to maintain clearance, look over both shoulders, back straight out until clear.

MACHINE SHUT-OFF AND EXIT
-
Park on level surface, lower lift arms and bucket to the ground.
-
Place controls in neutral, set the parking brake, turn off engine
-
Dismount machine by facing seat and using handholds and steps.
-
Maintains three points of contact when dismounting. Does not jump off of machine.
EVALUATION

Based on my evaluation, the operator has completed the evaluation and is qualified to operate the equipment
