Observer Guidlines

  • 1) Observe one employee per card
    2) Observe one work activity per card
    3) Observation period 5 - 10 minutes
    4) If unsafe activity is observed, discussion with employee is required
    5) Provide positive (4:1) and immediate feedback to employees
    6) Comments section should include ideas for improvement

Line of Fire

  • Required Personal Protection is being utilized correctly (hand, body, eyes/face, head, foot, respiratory, fall protection, hearing protection, or FR clothing/gear)

  • Seat belt compliance

  • Eyes on task ( no distractions, etc cell phone usage....)

  • Required safety training completed (company and customer)

  • Proper housekeeping (site order, equipment cleanliness)

Initial Actions

  • Check if JSA/Safety Orientation is complete

  • Is employee paying attention to the work activity

  • Proper body language (balance, trip hazard, slip hazard)

Body Mechanics (Ergonomics)

  • Working Position: poor posture, over stretched, above shoulder, below knees, twisting, caught in between, struck by, working at heights greater than 4', floor or wall opening, unprotected work edge, or false ceiling or floor

  • Proper lift, bend, twist (awkward, 3 point position, bending at knees, etc...)

  • Repetitive motion activity

  • Reaching, pulling, pushing, exceeding normal body limits

  • Standing, sitting, kneeling for long durations

Tools and Equipment

  • Appropriate tools or equipment being used

  • Safe use and operation

Work Activity

  • Position of employee (exposure to Stuck by, Caught by, Caught in Between, Temperature, Inhalation, Fall Hazards, etc...)

  • Confined Space compliance

  • Isolation Lockout, Tag-out compliance (electrical, mechanical, equipment)<br>
