Jobsite Amenities, Posters and Signage

  • Are fencing, Jobsite safety signage and barricades in place as necessary to protect both employees and the public?

  • Are traffic controls in place as required for work in congested areas or adjacent to roadways?

Erosion Control

  • Are all required SWPPP inspection reports up to date with all repairs and modifications completed and documented on the Erosion Control Plan?

Fire Protection

  • Are fire extinguishers readily available and current on annual and monthly inspections?

  • Are gas and diesel stored in approved metal safety cans, as defined in 1926 Subpart F?

Personal Protective Equipment (PPE)

  • Do you strictly enforce the CMSWillowbrook standard PPE policy requiring safety glasses, hard hat and CMSWillowbrook (or company specific) high-vis upper garment with prominent logo that is visible from 50' or more?

  • Is other PPE, whether or not specifically listed, utilized as required for other tasks?

General Fall Protection (Excluding Ladders and Scaffolds)

  • Are walking surfaces free of debris, oil and other substances that are likely to cause slips, trips and falls?

  • Are stairs, ramps and ladders utilized when and where needed?

  • Are leading edges, floor holes, floor & window openings, skylights and similar fall hazards guarded?

  • Is roof fall protection in place to protect rooftop workers?

  • Are steps, ramps and landings equipped with guardrail systems where required, including at storage trailers?

  • Have subcontractors been reminded of their contractual obligation to ensure all Personal Fall Arrest Systems (PFAS) are inspected by and used under the direction of a competent person?

Impalement Hazards

  • Are rebar and other impalement hazards capped or guarded?

Concrete and Masonry

  • Is the masonry lay-down area free of impalement hazards, or is it barricaded to prevent employee exposure?

  • To the extent feasible, is the wet-cut method used when sawing concrete, masonry products and similar materials, including cuts made with a Quickie-saw?

Scaffolds

  • Have subcontractors been instructed to ensure their scaffolds are inspected by a competent person each morning and as work progresses to ensure safety compliance is maintained?

  • Have non-compliant scaffold sections been tagged or barricaded to prevent access by anyone other than erectors or dismantlers?

  • Do scaffolds that are in use appear to be OSHA compliant and used in a safe manner?

Ladders

  • Do ladders appear to be of the approved type, legibly labeled and used in accordance with the manufacturer's instructions?

Electrical

  • Is temporary power fed through GFCI's which have been tested at least monthly?

  • Do extension and power tool cords and appurtenances appear to be in good condition and of the approved type?

  • Are Lockout-Tagout measures in place to ensure live electrical panels and parts are accessible only to qualified persons?

  • Have you advised subcontractors that all switches, receptacles and similar devices are to remain de-energized until cover plates are installed?

  • Are signs utilized as required to designate circuits that are de-energized for working downstream, and have those circuits been rendered inoperable, either by locks or by disconnecting the wiring from the power source?

  • Does general lighting appear adequate?

Hand and Power Tools

  • Do tools appear to be maintained in good condition, properly guarded and used as designed?

Aerial and Scissor Lifts

  • Do aerial and scissor lifts appear to be adequately maintained and used in a safe manner?

Forklifts

  • Do forklifts appear to be adequately maintained and used in a safe manner?

Cranes

  • Has a Notification of Site Conditions for Cranes been issued by the controlling contractor?

  • Are the operator's daily log and the most recent crane inspection records on site and available upon request?

  • Has the work zone been identified and demarcated with signs, barricades or similar methods as required to maintain at least 20' clearance from overhead power lines?

  • Is an OSHA compliant encroachment plan in place if the work zone has not been identified and effectively demarcated to maintain at least 20' clearance from overhead power lines?

  • Do hooks, slings and other lifting equipment appear to be tagged and in good condition?

  • Are measures in place to ensure only qualified riggers and connectors will be in the lift zone during hoisting operations?

  • Do cranes appear to be adequately maintained and used in a safe manner?

Mobile Equipment

  • Does mobile equipment appear to be adequately maintained and used in a safe manner?

Excavation

  • Have subcontractors been instructed to ensure all excavation work is OSHA compliant and performed under the direct supervision of a competent person?

  • Do excavation competent persons appear knowledgable and diligent in regards to safety?

Welding and Cutting

  • Does damaged welding cable insulation appear to be repaired with rubber and friction tape, or equivalent insulation and free of repairs within 10' of the electrode holder? (No vinyl electrical tape, except when used as a very thin protective covering for rubber or friction tape.)

  • Are welding lugs protected by lug boots or equivalent means?

  • Are stored oxygen and acetylene bottles secured and stored at least 20' apart or separated by a 1/2-hour fire partition?

  • Are oxyacetylene torches equipped with flashback arresters?

Safety Supervision

  • When a subcontractor foreman change is made with an individual who was not involved in the initial Minimum Project Safety Standards review, is it your practice to review it plus all known and pertinent site specific safety issues with him prior to commencement of his competent person role?

  • Do you hold subcontractor foremen responsible for safety compliance relating to their work, employees and subcontractors?

Miscellaneous

  • Other than those required for safety meeting and SWPPP report verification, have you attached at least five safety verification photos to this report?
