  • Is the Safety Professional maintaining a daily log and is it up to date:<br>A. Are they using the TSC daily log wIth the Phase, Cause & Exposure codes included. <br>B. Does the log reflect the job-site conditions? <br>C. Is it balanced with both positive and negative observations?<br>D. Do their log entries contain the following: <br>1.The date the unsafe acts or conditions was first observed<br>2. Are they including the responsible party.<br>3. the date the unsafe act or condition has been corrected.

  • Does the Safety Professional have in their possession or access to the design drawings for equipment, netting (horizontal & Vertical) Sidewalk shed, Perimeter Fencing and other temporary platforms?

  • Has the Safety Professional conducted incident investigation and reporting?<br>Are they filling out TSC accident report/TSC Supplemental Report and giving a full description of the incident <br>(Including Who, What, Why, Where and How)<br>

  • Is the Incident log is current, detailing man hours and OSHA recordables - using the updated version 4/15/12.

  • Is a permit log being maintained. Does the Safety Professional have access to the permit log. Are all permits are up to date?

  • 8. The Safety Professional is performing new worker orientations on site and certifications are being collected?<br>a. Orientations on file.<br>b. Training Certs (OSHA/ 4hr User) on file.<br>C. Is an Orientation Log being maintained.

  • Is the Safety Professional is collecting and filing Competent Persons/ Safety Representative/ Concrete Safety Manager forms from the contractors/ subcontractors.?<br>Are they updating this information on the regular basis?

  • Are the Sub-Contractors handing in Toolbox Talks and the Daily Huddles on the weekly basis?<br>Is the Safety Professional maintaining a Matrix to track this paper work. <br>Has the GC/CM been notified of delinquent Subcontractors.

  • Is the job a Site Safety Project? <br>Does the plan match the site conditions?<br>Did TSC do the SSP

  • Is the Safety Pofessional getting copies of equipment checklist from the trades (Scaffolds, Cranes, Earth Moving Equipment, Aerial lifts)

  • Is the Safety Professional performing sidewalk shed inspections. Is their a file up to date?

  • Is the Site Safety Manager performing a dual role with Fire Safety or is there a separate Fire Safety Manager?

  • Is the Safety Professional performing severe weather checklist when weather alerts are sent? Is their a file up to date?

  • Is the Safety Professional maintaining an electronic copies of all their paper work (Scanning TBT, Orientations, Sidewalk shed, etc)

Construction Site Fire Safety Manager Paperwork

  • List all of the Safety Certifications and are they all current?

  • Is the CSFSM in the 18-month apprenticeship?<br>Are their monthly summaries being maintained and up to date?

  • What logs are the CSFSM maintaining?

  • Is the Fire Safety Manager supervising the issuance of hot work permits for all hot work operations?<br>Are the hot work permits being returned and closed out at the end of the day?

  • Is the Fire Safety Manager monitoring operations of the Fire Guards?<br>a. Current/ Appropriate Certificate of Fitness for all Fire Guards<br>b. Fire guard log book.

  • Is the Fire Safety Manager maintaining a record of all Certificate of Fitness being used on the job?

  • Is the Fire Safety Manager maintaining proper signage throughout the job-site (Exit, Fire extinguishers, stair signage and emergency evacuation signage)

  • Is the Fire extinguishers inspections up to date? Are the correct amount of fire extinguishers being maintained by the GC/CM

  • Has the Fire Safety Manager performed basic fire safety training? Has personnel been being briefed with basic fire safety practices, procedures and how to use a fire extinguisher during the new worker orientation.

  • Has there been any Emergency Evacuation Drills?

  • Is the Fire Safety Manager maintaining and filing copies of Emergency Evacuation Plan and the Pre-fire Plan?<br>Are these plans current?

  • Is the Fire Safety Manager tracking all FDNY permits? Are they up to date? <br>Where are the cabinets and cages being stored?

  • FDNY First Responders Box: The Construction Manager has established at a location readily accessible to fire personnel responding to a site emergency. <br>Are the contents of the box being maintained on the regular basis?
