TAI Safety Policies / Working Conditions

  • Is Lock-out / Tag-out being followed in ALL areas of the plant?

  • Are there Work Instructions / Standards for the task being performed?

  • Are all TM's wearing the required PPE for the given task?

  • Were all equipment / tools (cranes, wire ropes/nylon slings, forklifts, etc...) inspected and a check sheet completed prior to use?

  • Coil Storage: Are coils stored properly? (No suspended coils, heavy coil stacked on a smaller coil, too wide for the row, etc...)

  • Is the "3 meter" forklift standard being followed?

Behavior Based Risk Safety

  • Are ALL employee's walking in a designated walking path? (Except when working on processing lines or WH)

  • Are ALL employee's observing to not walk between coils?

  • Are TM operating Cranes on 45 degree rotations and not operating with Cranes behind them?

Employee Observations

  • Are TM's wearing all applicable PPE?

  • Are TM's using PPE properly (i.e. eye protection on head, gloves in back pocket)?

  • Are TM's placing themselves in dangerous areas? (i.e. near moving parts, reaching inside machines, standing in known hazard areas)

  • Are TM's using the proper Hand Tools, are Hand Tools set up with good ergonomic design?

  • Are TM's operating with unsafe behaviors?

  • Are TM's using the proper tools to conduct the task? (i.e. crow bar being used as a hammer)

  • Can TM explain the safety topic from pre shift meetings?

  • Are TM's conducting/practicing proper ergonomic lifting protocols?

Floors and Aisles

  • Are floors and aisles clear of Slip & Trip Hazards?

  • Do floors and aisles have designated walkways clearly marked and free from stored and staged materials?

  • Is a daily 5-S being conducted on floors and aisles?

  • Are barrier guards in place protecting man and machine from industrial vehicle traffic?

  • Is there sufficient room for forklift traffic and maneuverability?

  • Are Surfaces non-slick in nature, and where slick (i.e. water, oil, hydraulic fluid) posses slip resistant flooring (i.e. grating, textured)?

  • Is there secondary containment in areas where oils, fluids, greases are stored?<br>

Exits

  • Do Exit Doors Require Special Knowledge or tools to open (i.e. keys, padlocks, latches)?

  • Are the Emergency Routes Posted?

  • Are Exit Doors Labeled with an Exit Sign, and are Exit Signs illuminated?

  • Are Exits free from 360 Degrees of obstruction and 3 feet from any path way leading to the Exit?

  • Are doors that could be mistook for an Exit Labeled "Not An Exit"?

Ladders and Stairs

  • Are Portable Ladders and Portable Stair Units free of Damage?

  • Do all Fixed ladders 20ft or more in height possess a cage encircling the top?

  • Are Ladders and Portable Stairs in good working condition (i.e. casters roll properly, step locking actuators perform correctly)?

  • Are all Ladders maintained in a safe condition and stored in a secured and safe fashion (i.e. bungee corded or chained to structure)?

  • Do all surfaces with a 4 foot or more drop to the next level possess a rail system that includes a top, and mid rail? If TM work below the top level is there a kick plate to prevent items from falling and striking TM's?

  • Do stairs with 4 or more Risers (4 Steps or more) have handrails or stair railings?

Machinery and Equipment

  • Are all Safety Devices functioning properly (i.e. interlocks, fences, light curtains, etc.)?

  • Are Cranes Marked with Visible weight limits?

  • Are C-Hooks marked with visible weight limits?

  • Are Grabs Marked with Visible weight limits?

  • Where electrical forklifts are in use, is there a battery filling station with proper PPE, and eye wash within close proximity?

  • The Danger Zone on or around Machinery is clearly marked, defined, or identified?

  • Is there a Preventive Maintenance Schedule for machines and is it being maintained?

  • Does Machinery have Lock Out Tag Out procedures clearly posted on the machine by the lockout point?

  • Are Work Instructions posted or available for all jobs?

  • Are Daily inspections on machines, industrial equipment (i.e. forklifts, crane, safety interlocks, light curtains) being conducted and tracked through checklists?

  • Is the Machinery and equipment maintained in a good 5-S condition?

Material Handling and Storage

  • Are items stacked and stored in a way to prevent the from falling?

  • Are racking systems/storage racks secured in place?

  • Is the max weight limit posted for the max load of each shelf?

  • Are Pre-Shift Inspections Conducted? (i.e. Slings, chains, forklift attachments)

  • Are push, pull carts constructed properly and labeled? (i.e. Push Only, Pull Only))

  • Are racking systems/storage racks damaged?

  • Are hazardous chemicals stored in designated areas and away from high traffic areas?

Shipping and Docks

  • Are box trailer chalked when they are being unloaded?

  • Are Trailers chalked and or dock locked prior too loading and unloading?

  • Are cones and flags deployed outside in front of trailers and inside in the work zone?

  • Are keys taken from drivers when a live load situation is presented?

  • Are Docks painted according to the TAI Dock Template?

  • Is the Dock Safety Notice Sign visible for all to see prior to entering lot?

  • Are Trailer Pads free of damage? (i.e. holes, ruts, broken concrete)

  • Are Docks numbered and labeled?

  • Are Dock Lights visible?

  • Are Jack Stands in place for use, and do current dropped trailers have the appropriate Jack Stands in Place?

  • Is there a trailer check in place before entering with forklift?

  • Are glad hand locks available for use and in use where dock locks are not available?

  • Is there a Dock Lock Maintenance Program?

Maintenance Shop/Parts

  • Are Flammable cabinets closed and firm on ground?

  • Are Hand Tools and equipment stored in their proper locations?

  • Does the Drill Press have a chuck guard attached?

  • Does the Lathe have a spindle guard, and chuck guard attached?

  • Are all machines secured to the surface they occupy?

  • Are SDS sheets available in areas where hazardous chemicals are used and stored?

  • Is the Maintenance area well ventilated for weld, cutting, brazing activities?

  • Are Gas Cylinders properly chained to prevent tipping over?

  • Is maintenance shop clear of debris and in good working order?

  • Are Fire Extinguishers present in the shop and maintained?

  • Are guards in place for all hand tools being used?

  • Are Hand tools designed to prevent fingers and hands from being crushed, amputated, lacerated, or smashed?

  • Are Maintenance areas 5-S

Mobile Equipment

  • Are TM's who are operating forklifts wearing seat belts?

  • Are TM performing Pre-Shift Inspections?

  • Do TM operating aerial lifts don harness and lanyards at all times?

  • Do mobile equipment operators have an up to date license?

  • Do TM's know Emergency Protocols for operating industrial vehicles during power outages?

Electrical

  • Is there visible damage to electrical cords, or conduit about equipment? (i.e. electrical tape on cords)

  • Are extension cords being used as a permanent power source? (i.e. zip tied, mounted, taped, run under floors)

  • Are fans plugged into receptacles and not extension cords?

  • Are Red Emergency Stop Buttons free of damage or cracking?

  • Are electrical circuit breaker boxes guarded from accidental damaged, have labeling for arc flash testing, and with the voltage traveling into the box?
