NG Policy and Procedures

  • Is there suitable personnel on site for the completion of the task, including a certified CP able to accept a FOA/PtW or PtW for Excavations?

  • Is there a requirement for a Form of Authority, Permit to Work or Permit to Work for Excavations?

  • Is the permit in date, has it been signed by both the AE and CP and does the permit cover the works required?

  • Does the location on the permit match the location on site?

  • Has the permit been registered with Network Control?

  • Have the team got the relevant qualifications on site e.g. Street works/NG competencies/digger driver tickets?

  • Within the RAMS does it specify the nearest Accident and Emergency department?

  • Does the team have a reporting process for Accidents, illness and injury, does it involve reporting to National Grid?

Health and Safety

  • Are vehicles parked safely, 5M upwind of the excavation point?

  • Are safety barriers required, if so has thought been given to allow the safe passage of MoP's through the site?

  • Is there adequate signing, lighting and guarding for the size of the excavation?

  • Is traffic management required? If so is there an adequate traffic management plan in place?

  • Has a site specific risk assessment been carried out and all hazards identified been controlled?

  • Have all utilities mains and services been surveyed using a Cable Locator tool on all 3 modes and marked around the area of the excavation using the correct coloured spray?

  • Does the team have all of the relevant utilities drawings on site to assist them with location and are they all up to date I.e within the last month?

  • Is there an area site aside upwind 5M from the excavation whereby all tools and equipment can be stored safely?

  • Is the First aid kit available and are the items within in date?

  • Is there 2 x 500ml bottles of eyewash available on site?

PPE

  • Is the BA system in good working condition and has the monthly check sheet been carried out, signed and dated?

  • Are the team wearing suitable PPE for the task, including flame proof overalls, high viz waist coat, safety boots, safety glasses and gloves suitable for the task?

  • Is there any special PPE requirements e.g. Full face visor for use with Stihl Saw?

  • Is the special PPE in good condition for use and where applicable in date, e.g. Hard hats?

  • Is full High Viz clothing required, for works on roads where the speed limit is greater than 40mph?

  • Is Full High Viz being worn?

  • Does the man within the excavation have a personal gas alarm? Is it calibrated and in date?

  • Enter any comments regarding the state of the PPE being worn and any further recommendations.....

Excavation

  • Is there requirement for a section 50 streetworks notice in place and is the permit number visible/displayed on site?

  • Has the area to be excavated been thoroughly swept using a CAT and Genny in all 3 modes, and and utilities marked?

  • Is the relevant permit in place, signed and dated correctly?

  • Has the area been swept with a GASCO seeker to ensure the atmosphere is safe to work in?!

  • Has the area been damped down to reduce the risk of sparks?

  • Has the area been reswept using the CAT and Genny in all three modes every 300mm in line with HSG47 Safe Digging Practices?

  • Is a banksman required, (Mechanical Excavators only), if so is there evidence of suitable training?

  • If Mechanical Excavation is required is the driver suitably trained for the type of machine being used?

  • Is there evidence that the Mechanical excavator has been used within 500mm of any utilities?

  • Is the spoil a minimum of 300mm from the edge of the excavation?

  • Are there any tools and equipment stored around the edge of the excavation that are listed kept to cause a risk of injury through falling into the excavation?

  • Is the excavation suitably barriered off?

  • Is the excavation deeper than 1.2M? If so, is the permit on site correct for the work being undertaken and is it being adhered to?

Tools and Equipment

  • Does the supplied Stihl Saw come with a suitable water bottle to surprise any generated dust?

  • Is the GASCO seeker/Bascom Turner unit calibrated and in date, is there sufficient battery life for the works required and is the housing of the unit in good condition?

  • Is the Cable Location tool and generator calibrated and in date, is there sufficient battery life for the works required and is the tool in a suitable condition?

  • Are there 2 x 9kg dry powder Fire Extinguishers on site and serviced?

  • Are the fire extinguishers properly pressurised?

  • Is the Mechanical excavator being used fit for purpose and is it in a suitable condition for use?
