  • Are all lifting accessories being used under current certification? e.g. slings, spreader bars.

  • Was the hazard identification(docket) or Risk Assessment on lift plan been accepted and signed by the person in charge of the work being carried out?

  • Has the docket book been filled out correctly and is the worker recording lost time?

  • Was a lift plan completed prior to starting the task? and was the lift plan signed by all parties involved in the lift?

  • Is a truck manual available upon request? is the worker familiar with this? and is it the current version?

  • Are the permits for the truck available upon request? e.g. 20m permit for IRCH Truck 5, and are the appropriate placards being displayed e.g. H for overlength

  • Are the truck and trailer loading certificate readily available?

  • Check registration, COF, registration and RUC, are they current? To be sighted to verify.

  • Does the operator hold the current training qualification to be operating Truck/Hiab? e.g. Hiab: US 16617 & 25043. If not, is the operator being trained under the IRCH/TMCH Buddy Training System?

  • Is the employee familiar with the emergency procedure? and are these displayed in the vehicle e.g. truck safety manual.

  • Is the worker wearing the correct PPE and is this in a good condition? e.g. Hard Hat, Safety Specs, Footwear. Consider site specific requirements.

  • Is the date on the worker(s) hard hat within 2yrs of date of issue?

  • Nutrition: Is the employee aware of the need for correct food and adequate water consumption?

  • Are seatbelts installed in the vehicle? are these being worn when required?

  • Does the operator know where the emergency equipment is kept? e.g. First Aid kit, fire extinguisher, spill kit? Are these all in a "fit for purpose" state?

  • Are rating charts available for Hiabs?

OPERATIONAL

  • Has the operator completed a pre-use inspection?

  • Does the Hiab have one or both of the following: safe load alarm/cut out or safe load indicator or load moment indicator?

  • Are all the gauges in working order?

  • Is the operator comfortable with the instrumentation e.g. do they know what all the gauges are for?

  • If the Hiab is working within 4m of powerlines, is there a permit for the work? verify this by sighting permit.

  • Is the work area sufficiently controlled from other work parties? e.g. by use of barriers.

  • Does the operator control the lift in a smooth and efficient manner?

  • Is the vehicle in a good condition? e.g. operational, clean and tidy?

  • Is the operator aware of the vehicle servicing arrangements? e.g. oil changes, greasing, when are they due etc?

  • Is there danger tape readily available?

  • Did the operator ensure the load was secured appropriately before lifting?

  • Is the operator familiar with the correct use of cross lock and diff locks?

  • Are general driving habits ok? e.g. speed, courtesy, following distance, driving to conditions etc.
