Have all personnel engaged on the task signed onto the JSA

Has the JSA been reviewed and signed by the Job supervisor.

In the case of a lower tier subcontractor has the JSA been authorised by the principle contractor

Does the JSA Identify the task being performed when u arrived at the location.

Does the JSA Adequately reflect the task being performed.

Does the JSA have an overriding Hazard assessment.

Are the JSA task steps being followed

Have all potential hazards been identified ( detail major hazards that have not been accounted for)

Are Critical controls and the STOP CRITERIA listed.

Does the JSA reflect any field modifications.
