  • Hollow pan stairs - filled or have barricade up?

  • Proper handrails at stairwells?

5) SCAFFOLDING

  • Are working areas free of debris, snow, ice, and grease?

  • Are workers protected from falling obhects?

  • Are guard rails, intermediate rails, and toe boards in place?

  • Are ropes and cables in good condition?

  • Work platform - fully decked?

  • Are proper access ladders being used?

  • Scaffolding inspected daily? (Tags present?)

  • Baker and Perry scaffold fall protection at 4 feet?

6) BARRICADES

  • Are floor & roof permieters and wall openings guarded?

  • Proper signage or Flaggers/Paddlers?

7) EXCAVATION AND SHORING

  • Are adjacent structures properly shored (including roads & walks)?

  • Is there proper protection? Sloped/Benched/Shored/Shielded

  • Is material 3' from edge?

  • Is excavation barricaded?

  • Are ladders proveded and used?

  • Environmental Conditions: Dust Control, Silt Fencing, etc.

8) FALL PROTECTION SYSTEMS

14) DEMOLITION

  • Is there shoring of adjacent structions?

  • Is there sidewald and other public protection?

  • Adequate access to ladders or stairs?
