Questionnaire

The version control will be maintained by the SHEQ team and is controlled via the Safety Equipment Inspections Document 2073838157-15.

The following section details job steps and associated identified hazards which, without controls, have been identified as High or Extreme Risk. It is the candidates responsibility to detail the controls they believe to be appropriate as part of this process.

Considering the pre-start inspection regime of the equipment, please detail the controls you believe should be used in relation to the following identified hazards:

  • Electrocution through contact with overhead electrical assets

  • Has the candidate provided appropriate responses?

Considering the task of lifting a load with a hydraulic vehicle mounted crane, please outline controls for the following hazard(s):

  • Failure of lifting device through overloading

  • Collision between vehicle mounted loading crane and other heavy plant

  • Dislodgement and falling load causing potential injury to personnel

  • Spill of substance while lifting equipment

  • Pinch or crushing injuries to crane operators

  • Injury or plant damage through people/plant or plant/plant interactions

  • Fall from height of personnel while operating VLC near unprotected edges (eg Well openings, manholes or reservoir access points)

  • Conducting lifting operations in high wind velocities

  • Illness due to working in extremes of temperature (eg hypothermia, heat stress/heat stroke)

  • Poor visibility due to low light (or no light) conditions

  • Has the candidate provided appropriate responses?

Considering the task of performing maintenance to the vehicle mounted crane, please outline controls for the following hazard(s):

  • Unexpected movement of equipment potentially causing injury to person

  • Burns or scalds from contact with hot equipment

  • Has the candidate provided appropriate responses?

Considering the various emergency response scenarios an operator may be faced with, please outline controls in the following situations:

  • Personal exposure to a Chemical/fuel or oil spill

  • General site emergency alarm

  • Fire or explosion

  • Has the candidate provided appropriate responses?

Acceptance: By signing this LOUQ both parties are recognising that either all questions have been answered appropriately or discussions have occurred in relation to any incomplete controls.
