INSPECTION CHECKLIST

Work Health and Safety Management Plan

  • The names, positions and responsibilities for those that have specific health and safety responsibilities, are documented?

  • The arrangements for consultation, co-operation and the co-ordination of activities in relation to the works have been documented?

  • The arrangements for the management of subcontractors, including monitoring activities, have been documented?

  • The arrangements for managing work health and safety incidents have been documented?

  • Site-specific health and safety rules have been documented?

  • The arrangements for the collection, assessment, monitoring and review of SWMS at the workplace have been documented?

Site Induction

  • A project specific site induction, which includes reference to the WHS Management Plan (where applicable) and Centre requirements, has been developed and implemented?

  • Records are available to confirm that inductions have been completed and Construction Industry Induction has been verified?

  • The Vicinity Centre Hazardous Materials Register is available and was discussed with workers as part of the site induction?

Risk Management and Safe Work Method Statements

  • Safe Work Method Statements (SWMS) are available for inspection?

  • SWMSs are site specific, current, appear to address relevant risks and are signed off by relevant workers?

  • The Principal Contractor can demonstrate review of sub-contractor SWMSs and/or other risk management documentation?

  • Current risk assessments and Safety Data Sheets (SDSs) for the storage and handling of significant quantities of hazardous chemicals are available for inspection?

  • Vicinity Centres permit to work processes are being complied with?

Incident Reporting and Investigation

  • Incidents have been reported to Vicinity Centres and have been investigated as appropriate?

  • Relevant workers have been consulted in incident investigations?

  • There is evidence of review of control measures and follow up actions taken?

  • Notifiable incidents have been reported to Vicinity Centres and there is evidence of notification to the appropriate Regulator in accordance with jurisdictional requirements?

License and Competency Requirements

  • The Principal Contractor verifies and maintains licenses and competencies for relevant work activities, such as:<br>• Confined space<br>• Electricians licence<br>• Forklift licence<br>• EWP licence (required for boom type EWP that extends greater than 11m)<br>• Work at heights competency<br>

Site Inspection

  • Working at height activities appear to be adequately controlled?

  • The Principal Contractor can demonstrate that Scaffolds > 4m are erected and dismantled by a licensed contractor?

  • PPE appears to be worn in accordance with site requirements?

  • There are controls in place to ensure the site is secured from unauthorised access?

  • Traffic management protocols have been implemented?

  • The Principal Contractor has confirmed that significant quantities of hazardous chemicals (including gas cylinders) are stored appropriately?

  • Plant and equipment appears to be:<br>• In good condition;<br>• Subject to a regular maintenance schedule/pre-start checks;<br>• With safety devices/controls in place; and<br>• Stored appropriately?<br>

  • Portable electrical items and leads are electrically tested and tagged?

  • No uncontrolled significant slip, trip and/or fall hazards identified?

  • Appropriate emergency provisions, e.g. fire extinguishers, first aid facilities, and evacuation protocols, appear to be in place?

  • Principal contractor signage (mandatory for construction projects) is posted on the exterior of the worksite and includes:<br>• The principal contractor’s name and telephone contact numbers (including an after-hours telephone number);<br>• Shows the location of the site office for the project, if any; and <br>• Is visible from outside the workplace, or the work area of the workplace.<br>
