Walkie Forklift Truck Pre-Start Checklist

  • Is there any damage anywhere?

  • Check the forks; they are secure, not bent or cracked?

  • Check lift chains; free from damage?

  • Is the battery charged sufficiently?

  • Are the wheels in good condition?

  • Look under the forklift and around the mast hoses; free from oil or battery leaks?

  • Forklift starts ok?

  • The Power Disconnect works correctly?

  • Horn working?

  • Check all controls - do they work smoothly?

  • Check the raise and lower to the max height; operating smoothly and no defects?

  • The steering is smooth?

  • Operate slowly forward & back, and forklift operates smoothly in all directions?

  • The stopping distance is good in both forward & reverse?

  • Walk around the warehouse and outside & check the areas where the forklift will be operated – there are no objects in the way that will be a hazard?

  • The operators clothing is tucked in and no loose sleeves etc?

  • Cones, concertina barriers & warning signs are ready to place out when the forklift operating in warehouse and outside on driveway?

Review & Sign-Off

  • Walkie Forklift Truck is safe to use?

  • 1. I have applied a defect tag (LOTO) or warning sign?

  • 2. Keys have been removed & handed to Team Leader?

  • 3. You have notified the Team Leader or Manager?

  • 4. Arrange repair

  • Signature of person completing the inspection.

Team Leader Weekly Checks (To be completed weekly by the Team Leader or Manager)

  • Operators have been observed making the pre-start checks at least once in past 2-weeks?

  • Operators appear to be competent in making the pre-start checks on the Walkie Forklift Truck?

  • If not competent then additional training has been arranged or conducted?

  • Team Leader or Manager Name
