WAVE Pre-Start Checklist

  • Is there any damage anywhere?

  • Is the battery charged sufficiently?

  • Are the wheels in good condition?

  • Free from oil or battery leaks under the vehicle? (check under and around the vehicle particularly around the mast hoses)

  • Vehicle starts ok and no warning indicators displayed?

  • Turn the key on: The bleeper and all display lights are working?

  • The Emergency Disconnect button works correctly?

  • Horn working?

  • The service panel door is closed?

  • Check the raise and lower (with gates open). The platform did not elevate above 20 inches with the gates open?

  • The steering is smooth?

  • Vehicle drives well in all directions?

  • The vehicle drives slower with the gates closed? (check speed when gates are open & closed)

  • The stopping distance is good in both forward & reverse?

  • Walk around & check the areas where the WAVE will be operated – there are no objects in the way that will be a hazard?

  • The operators clothing is tucked in and no loose sleeves etc?

  • Tools are secured to operator to prevent falling at height?

  • Cones & warning signs are ready to place at aisle ends when WAVE operating down aisles?

Review & Sign-Off

  • WAVE is safe to use?

  • 1. I have applied a defect tag (LOTO) or warning sign?

  • 2. Keys have been removed & handed to Team Leader?

  • 3. You have notified the Team Leader or Manager?

  • 4. Arrange repair

  • Signature of person completing the inspection.

Team Leader Weekly Checks (To be completed weekly by the Team Leader or Manager)

  • Operators have been observed making the pre-start checks at least once in past 2-weeks?

  • Operators appear to be competent in making the pre-start checks on the WAVE machine?

  • If not competent then additional training has been arranged or conducted?

  • Team Leader or Manager Name

  • Date / Time

  • Signature
