Working at Heights

  • Persons working at heights must have documented evidence of proper training. Project requirements include 100% tie off at all time while working at height.

  • Full body harness worn in the proper manner

  • Lanyards are adequately secured to to suitable anchorage

  • Perimeter guarding or guardrails are in place to preclude injury from falling objects

  • Lifelines are installed to proper anchorages and can support 5,200 lbs. of force

  • Fall protection equipment is maintained in good condition with current inspections in place

  • Fall protection equipment is used only for fall protection

Excavations

  • Excavations include trenching as well as pits and are characterized by having limited access and egress and the potential for the engulfment of persons working within.

  • Trenches and excavations are sloped, benched or shored properly

  • Proper access and egress is provided

  • Trenches and excavations are barricaded appropriately

  • Trenches and excavations have documented inspections daily done by a competent person to ensure their integrity before entrance by workers

Scaffold and Ladders

  • Scaffolds are inspected and tagged by a competent person

  • Proper access and egress is in place

  • Scaffolds are correctly tagged

  • Proper ladder is used for the job performed and it is properly secured

  • Ladders are set at proper angle and extend beyond the landing (3 ft or 0.9 m)

Hoisting and Lifting Equipment

Vehicles and Heavy Equipment

Electrical Equipment and Tools

  • Electrical cords are in good condition and have current inspection

  • Tools are in good condition and electric tools have current inspection. There are no homemade tools

  • All machine guards are in good condition and in place

  • GFCI's are in place on all electrical equipment

Fire Protection and Flammable Gases

  • Flammable materials are properly stored in designated storage location

  • Oxygen and Acetylene tanks are properly separated in storage areas

  • Compressed gas cylinders are stored upright and secured

  • Gas regulator gauges are properly attached and flash arrestors are installed

  • Fire watch is in place when required

  • Hot work areas are contained and welding screen are in place

  • Fire extinguishers are properly located and have current inspections

Confined Spaces

  • Confined spaces include manholes, pits/tanks, pipes/culverts and other restricted access/enclosed spaces. A specific risk assessment carried out in compliance with the Permit to Work must be carried out. Only appropriately trained and qualified personnel are allowed to participate in confined space entries, including those with duties outside the confined space.

  • An appropriate risk assessment has been completed and the confined space entry permit is correctly completed

  • All control systems such as air monitoring, entry control lists, space ventilation systems, rescue systems etc, are in place and address the findings of the risk assessment.

  • Confined space permits are cancelled and closed at the completion of daily work

Environmental

  • All earth storage and disturbances are in compliance with existing storm water permits

  • Spill kits and bunding are in place at and around diesel storage tanks

  • Concrete washout facilities have capacities suitable to accommodate upcoming concrete pouring activities

  • Dust generation is minimized through control procedures

Demolition

  • All hazardous material removal contractors are trained and qualified in compliance with all applicable regulatory requirements

  • Hazardous materials are disposed using only approved transporters and disposal sites

  • Demolition activities are planned and supervised by qualified personnel

  • Demolition activities do not leave unstable structures in place at the completion of shift

  • Non-hazardous demolition wastes are disposed of in a timely manner and not accumulated

  • Appropriate barricading is in place to prevent entry of unauthorized personnel into demolition areas or areas where unstable debris may be present
