HEALTH & SAFETY

1. Formwork & Falsework - PRA, SWMS:15 Falls managed, exclusion zones & signs in place, joists placed from below, single leading edge.
2. Ladders & Trestles - SWMS - 01 Suitable for the tasks, correct setup inspected prior to use, removed from service if damaged.
3. Materials Handling - PRA, SWMS: 04 Are safe and correct mechanical aids or team lifting methods being uterlised.
4. Excavation - PRA, SWMS:07 - Permit HB - HSEQ - F - 078 Collapse controlled, barricades in place, safe access, stockpiles away from trenches.
5. Cranes & Lifting -PRA. Safe lifting practices near workers, warning processes or exclusion zones in place, tag lines being uterlised to control drifting loads.
6. Fire / Emergency Equipment maintained MP - 09 PRA. Extinguishers positioned at stair access/hoist, temporary access/egress, adjacent Hazardous & Dangerous Goods Storage areas. 6 Monthly test carried out.
7. Emergency Exits, Alarms, Signs - PRA. Air horn charged, exit signs (running man) erected on levels, back up emergency lighting maintained and tests recorded.
8. Concrete Pumps / Trucks Set Up - PRA. Pumps set up on stable ground, all obstructions cleared, exclusion zones established, traffic control in place (if pumping on public road), pump checklist/log books completed.
9. Traffic Management - PRA, SWMS:23. TMP developed to suit site and surrounding conditions, deliveries and public interaction scheduled and monitored, council and MUTCD in compliance.
10. Asbestos - PRA, SWMS: 09. Qualified licence held by operators, exclusions in place, Asbestos Management Plan, separation and disposal methods in place and recorded, regulator notified, all records in place including Asbestos Register.
11. Demolition PRA, SWMS:26. Demolition Plan on site, exclusion zones set up, methodology defined and identified, licences (including operators) permits & notifications identified.
12. Hazardous Chemicals & SDS Records - SP:01, PRA, SWMS 14. SDS & Risk Assessments supplied for Hazardous Substances and Dangerous Goods used on this site, Allocated areas for Hazardous Substances & Dangerous goods are bunded and in compliance, ALL Hazardous Substances & Dangerous Goods have clear identifiable labels affixed.
13. Confined Spaces - PRA, Permit - HB - HSEQ - F - 075. Rescue Plan/Procedure in place and tested, atmospheric testing equipment available and calibrated, workers trained and qualified in 'Confined Space' access to confined space is safe, spotter in place.

ENVIRONMENTAL

1. Noise & Vibration Controlled - SP: 02, PRA. Noise and vibration being monitored and records identified, PPE being worn, Working hours monitored for long term use where a vibration is identified as being a possible risk.
2. All Bunded areas are empty of Water - SWMS:06 Pounded water to be removed.
3. Spill Kits on Site & Maintained - EMP, PRA. Spill Kit contents are stocked & in good useable order, spills cleaned & incident reports submitted & waste disposed of accordingly.
4. Fauna & Flora - EMP, PRA. Protected Species identified and protection methods in place and recorded.
5. Neighbour Correspondence or Complaints - EMP. Neighbour Correspondence & Complaints recorded, actioned, closed out and letter of distribution recorded in register.
6. Sediment Control - Not Identified Leaving Site - EMP, PRA. Sediment fences erected & maintained, record of maintaining silt socks, no visible identification of soil or materials on roadway or gutters, all sediment controlled methods installed as per requirements.
7. Waste Management - EMP, PRA. Adequate Waste Bins provided and scheduled removal in place. Waste segregation identified on site or through contractor & records maintained.
8. Weed Removal - EMP, PRA. Control methods in place to maintain weeds including future growth.
9. Contaminated Land - EMP, PRA. Contaminated Soils and Waste removed in accordance with plan.
