  • The first 3 sections of this audit should be done as part of a conversation with a relevant stakeholder within the contractor organisation. The conversation should include visibility of the contractor Construction Phase Plan, as well as any other supporting documents that the contractor is able to produce to show they are managing welfare provision.

If the contractor is unable to provide a relevant stakeholder to support the conversation, please complete the audit utilising the most recent version of the Construction Phase Plan, which is available from the CityFibre CDM team.

The final section is a site visit, either individually or alongside the contractor, to see the site, or sites, and ensure that this visit aligns with the information that has been provided to you as part of the conversation.

You will be required to provide notes for any 'yes/no' or 'rating' questions. These notes should be supportive of the answer you have given. For detail on ratings please see the 'Compliance Assurance Procedure OHSF-PR-08-01'.
