  • Has an emergency evacuation/rescue plan been developed and in place if the trench is deeper than 1.5m?

  • If the trench is deeper than 1.5 m deep is it battered, benched or shored or inspected by a geotechnical engineer?

  • Has suitable barricading been identified in the SWMS? Is it erected in accordance with SWMS?

  • What type of barricading has been erected?

  • Are there vehicles operating in the area?

  • Have barriers been established to prevent vehicles form entering the zone of influence?

  • Are there controls measures in place to prevent vehicle fumes from entering the trench?

  • Is warning signage displayed in prominent locations?

  • Is the barricading and signage in good condition?

  • For trenches deeper than 1.5 meters, is there a means of access/egress every 9 meters?

  • Is the trench spoil located as per the SWMS? (at least equal to depth)

  • Is there materials located within the 'zone of influence'?

  • Are workers required to cross the trench/es?

  • Are properly designed walkways or bridges with guard rails and kickboards provided? Are the trench/es near roadways or paths? Are they open at night? Are there flashing lights / reflective tape to mark the trench/es?

  • If spotters are used for adjacent to services are they positioned in safe location

  • Is the trench/es inspected by a competent person prior to commencing work or after an adverse weather event?
