  • There is a register (or similar) which identifies confined spaces and the need for persons to enter confined spaces is minimised through hierarchy of controls application

  • Workers involved in confined space work have attained competencies in accordance with the Australian training framework (or international equivalent) for; -Working in a confined space -Atmospheric monitoring of confined space -Supervision of confined space work)

  • Prior to entry a confined space permit is completed and authorised by a competent person. The permit includes an assessment of the hazards and specific controls required to manage hazards, tracking of persons entering the confined space, rescue plan requirements, emergency communication/contacts

  • Prior to entry, testing of atmospheric conditions by a competent person utilising calibrated equipment is required. Atmospheric testing will include: Oxygen, H2S, CO and LEL. Where there is a risk of atmospheric hazards, monitoring of the atmosphere will be continuous

  • A competent confined space sentry/standby person shall be located outside of a confined space at all times when the confined space is occupied. The sentry shall be trained in atmospheric monitoring and will be supplied with means of communication with entrants

  • All sources of energy that could harm workers in a Confined Space must be positively isolated

  • Conduct Visual Verification - that Confined space work is controlled as per the requirements of the above. Where rescue equipment has been established it is appropriate and would be effective for rescue if deployed

  • Conduct Interview with Supervisors and relevant workers are aware of the procedural requirements for planning, monitoring and undertaking work in confined space
